Reporting to the Young Adult Services Care Manager, the Rehab Practitioner with Young Adult Social Recreation & Wellness works as a key member of a community-based multi-disciplinary team to assist young adults with addiction and/or mental health concerns to achieve their personally defined goals are met within the Social Recreation & Wellness program. This process is accomplished through a strength based, cooperative approach designed to facilitate functional improvement and result in young adults obtaining and maintaining satisfying leisure participation as part of their personal recovery. In close collaboration with the Recreation therapists, the Rehab Practitioner with Young Adult Social Recreation & Wellness must meet the demands of providing psychoeducational and recreation groups in a constantly changing situations. Staff are expected to work effectively in evolving and challenging situations. In addition to providing social and recreation groups, the Rehab Practitioner will also participate in activities to develop and/or enhance groups and increase young adults' involvement in programs within Young Adult Service.

As a Rehab Practitioner, you will be an integrated member of an interdisciplinary team that facilitates the recovery process for individuals with addiction and/or mental health concerns. You will be responsible for bringing an active and creative approach to welcoming, engagement, screening and transition support that maintains the uniqueness of each individual within a wellness-oriented, collaborative service delivery model. You will work as a key member of a multidisciplinary team to assist individuals with addiction and/or mental health concerns. This individualized process is accomplished through a strengths based, cooperative approach designed to facilitate functional improvement and result in service recipients obtaining and maintaining satisfying roles within their communities as part of their personal recovery. In close collaboration with occupational therapists, mental health therapists and other team members, you must meet the demands and needs of constantly changing conditions, situations and people and are required to work effectively in difficult or ambiguous situations. In addition to providing individual services, you will also participate in activities to develop and enhance partnerships and synergies with community partners.

Bachelor's degree in Social Work, Psychology, Nursing, Occupational Therapy, Education, Physiotherapy, or Speech-Language Pathology. Minimum of two years' experience in addiction and/or mental health.

Current CPR certificate at the BLS level C. Minimum two (2) years' experience within the last six (6) years in a community addiction and mental health setting.

Bachelor's Degree in Recreation Therapy. Bachelor's of Arts Degree. Experience facilitating groups. Experience working with the young adult population (16-25).
