Edmonton Zone Continuing Care provides services through five main program areas: Home Living, Supportive Living, Facility Living, Transition Services and Palliative Care. We are looking for an Assist Head Nurse Team Lead to join our Continuing Care Access (CCA) team to provide team leadership and organization over the activities of the CCA program within Continuing Care-Edmonton Zone. This position provides clinical leadership within the operational framework of Continuing Care. The AHN Team Lead assists with consultation and coordination of daily operations to the Edmonton Zone Continuing Care Access Program, providing mentorship and support to the nursing staff. This position leads the team in day-to-day operations, evidence-based practice, quality client care, liaising with other stakeholders and programs, triaging client care, coordinating outcome-based practice, and assisting with organizing and orientation and reorientation of staff. Knowledge with building, implementation and evaluation of process work specific to CCA would be an asset.
As an Assistant Head Nurse, you will be responsible for staff assignments, patient flow and acting as a role model, clinical resource and advocate for nursing staff. In addition to providing frontline nursing care, as required, you will take a leadership role in the promotion of quality patient care and the implementation of operational priorities. Your in-depth knowledge and skills, advanced judgment and clinical experience, will assist in providing solutions for complex health care issues at all levels with patients, interprofessional teams, administrators and policy makers. You will play a key role in providing safe, quality patient and family centered care while reflecting the shared vision and values of AHS.
Completion of an accredited nursing education program. Active or eligible for registration and practice permit with the College of Registered Nurses of Alberta (CRNA). Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP). 5-7 years of experience and knowledge of the Continuing Care Program required. Knowledge of the Continuing Care Access program would be highly beneficial. Excellent organizational skills - to be able to prioritize and efficiently organize many details in a timely manner. Expert knowledge with Connect Care, Meditech and Case Management principles. Communication and assessment skills to work cooperatively and effectively with the healthcare team members, clients and their families in a fast-paced environment, identifying needs via tele triage, and resolving conflicts. Ability to identify and manage risk and to mitigate and diffuse those situations. Must be able to regularly review and update resources. Ability to update and create process workflows which effect patient movement within the Continuing Care program Ability to identify issues which can impact workload and processes in place. At the time of interview, you may be required to demonstrate your computer knowledge and proficiency by completing a hands-on assessment. You must demonstrate sufficient typing speed to meet workload demand and navigate through various computer programs including Microsoft Office (Word, Excel, PowerPoint), Genesys, Visio, Outlook, Sharepoint, Insite and Internet.
Baccalaureate Degree in Nursing. Demonstrated leadership and/or management experience. Must be able to work independently and within a team environment. Must be able to cope with high levels of stress, fluctuating priorities, and time sensitive deadlines. Experience with introducing new initiatives and working on change management.
