Description
The Care Manager is responsible and accountable for providing the direction, team leadership and organization over the activities of Seniors Health within a specific area(s) in support of the vision, mission and business plan of Alberta Health Services. The Care Manager, Seniors Health will develop and maintain strong working relationships with key stakeholders and demonstrate the leadership.
In this leadership position, you will be making key decisions and establishing and contributing to the achievements and goals of the Continuing Care, as well as be responsible for developing and maintaining a strong working relationship with key stakeholders, facilitating change management, promoting team development, teamwork and coordinating staffing and clinical work as identified by the Seniors Health Manager. You will be responsible for developing and maintaining an effective framework for the operations of patient care services in order to facilitate the delivery of efficient, effective and safe patient care. These duties may include addressing and resolving staff and patient concerns, assisting with audits and Human Resource investigations, conduct position interviews and travel to the sites throughout the portfolio to successfully meet the needs of patients, families and staff. As the successful candidate, you will have experience in demonstrating a client-centered approach, ethical decision making and present the ability to develop creative and clinical approaches to client and resident care. In addition to a competitive rate of pay, AHS currently has a Remote Retention Allowance in place to an annual maximum of $3,000. This allowance is non-pensionable and is payable on an hourly basis for all hours paid at the basic rate of pay.
Graduation from a recognized health designation. Three (3) plus years' experience in a healthcare environment within the last five (5) years. Current Basic Cardiac Life Support-Health Care Provider (BCLS-HCP). Minimum of one (1) year's working in a leadership capacity (ie: Charge Nurse or Team Lead). One (1) years' experience within the the last three (3) years working in Continuing Care or Homecare. Previous management within a Long-Term Care or Homecare setting. Experience working in Meditech and RAI HC.
Effective communicator with strong organizational skills, influential skills, and resident focused orientation, and commitment to providing quality services. Effective leadership abilities combined with a strong commitment to operating with the team environment and the ability to motivate and inspire others to achieve common goals. Leadership skills, including demonstrated ability and comfort with decision making responsibilities, coaching and teaching and the ability to inspire and build confidence in others. Demonstrated ability to manage change and create innovative solutions for complex and diverse issues. Demonstrated ability to manage human, financial and physical resources within an operating environment. Demonstrated ability to foster partnerships and to achieve organizational goals within an organization and ideally managing within a unionized sector. Demonstrated ability to plan, organize and manage short- and long-term plans and projects and deliver patient focused results.
Case management experience preferred.
