Continuing Care Counsellor - Designated Living Option Coordinator

As a Continuing Care Counsellor (CCC), you will be working closely with interdisciplinary professionals with a variety of backgrounds where you will provide a wide variety of coordination activities to support patients and their families to access Continuing Care living options. Using a collaborative approach, you will review applications, manage continuing care waitlists and coordinate placement processes. You will be guided by your education, experience, demonstrated skill level and AHS policies and procedures, while reflecting the shared vision and values of AHS.

Active or eligible for registration with applicable professional association or college.

Minimum 2 years of clinical experience working as an RN/RPN preferably in a home care, supportive living or long-term care environment. Experience with electronic documentation systems such as Meditech and Connect Care. Experience with completing RAI-HC or RAI-LTC assessments. Demonstrated excellence in written and verbal communication. The successful candidate must have the ability to work in a fast-paced environment. Must be highly self-motivated, adaptable, flexible and able to be a team player.

Experience with Rural Waitlisting and Vacancy Management. Case management training is preferred.
