Description
Bassano Health Centre is an acute and continuing care facility. Core programs at the Bassano Health Centre include 24-hour emergency, medicine, ambulatory care services, primary care clinic services, and community health. Bassano Health Centre has 4 acute care, and 8 continuing care beds. As a member of the CoAct team, you play a key role in resident care planning (interdisciplinary including resident and family) and allowing residents to achieve their goals and enhance their quality of life. This environment is suitable for candidates who have a strong interest and skills in providing care to the elderly client. As a Health Care Aide (HCA), you will assist in providing a clean and safe environment. Monitor resident progress and report relevant information to the Registered Nurse in a timely manner. Monitor and participate in RAI MDS resident tracking, care plan development and implementation as directed. Following demonstration of competence, the HCA is responsible for providing restricted activities deemed appropriate by the organization based on the resident/patient condition and under the supervision of the Registered Nurse. Responsible for maintaining accurate, concise documentation that meets organizational legislative, and legal requirements. Bassano is in Alberta, Canada. It is located on the Trans-Canada Highway approximately 140 km east of Calgary and 160 km northwest of Medicine Hat.
As a Health Care Aide (HCA), you will work as a collaborative member of the multi-disciplinary team providing personal assistance including supporting activities of daily living and support services to patients (clients, residents) who require short-term assistance or ongoing support, in accordance with the patient care plan and facility policies and procedures. In this position you will work closely with patients, families, and caregivers spanning the continuum of care in home, community or health care facilities assisting nurses with the provision of routine care to meet patients' hygiene, nutrition, mobility, recreation and safety needs, assisting with organized activities or programs and observing and reporting on a patient's mood, health, and wellbeing. Working under a regulated health professional you will be assisting in the maintenance of a safe environment for patients, staff and visitors, and maintaining effective communication with patients and health professionals.
Completion of Health Care Aide Certificate within 18 months of hire and active directory status on the Alberta Health Care Aide Directory. Must provide a confirmation letter as proof of enrolment from the Directory at https://www.albertahcadirectory.com/health-care-aides/confirmation/. Nursing students currently enrolled in an approved Alberta nursing program who have successfully completed a minimum of 100 hours of practicum; and are either in their 1st year of practical nursing program; or 2nd year of baccalaureate nursing or psychiatric nursing program will be considered. Must attest to this compliance and provide proof of qualification.
Current BLS. Supportive Pathways certificate. NVCI training.
Minimum of one (1) year experience as a Health Care Aide. Supportive living experience preferred. Familiarity with Continuing Care Health Service Standards. Experience caring for patients with Dementia.
