Description
Located within the Rocky Mountain House Health Centre, you will work with a dynamic, supportive team and will have the opportunity to make a difference in the lives of people in your community. Rocky Mountain House is located 77 km west of Red Deer in west - central Alberta, on the North Saskatchewan River. This multicultural community has that small town feel with big city amenities. Rocky Mountain House has an active and helpful welcoming committee with the mission to assist all newcomers to the area in feeling welcome and becoming engaged and included in our vibrant community As a Licensed Practical Nurse, you will report to the Home Care Manager and will be responsible to assess, plan, monitor and evaluate clients in consultation with the Registered Nurse Case Manager. You will be responsible for coordinating and providing quality care to clients in the community and will provide direct care in partnership with clients and their families in their own residence or an alternate care setting, as determined by the assessment.
As a Licensed Practical Nurse (LPN), you are part of an interprofessional health care team utilizing nursing processes, critical thinking, problem solving and decision making skills. You will be guided by your education, experience, demonstrated skill level and AHS policies and procedures. You will play a key role in providing safe, quality patient and family centered care while reflecting the shared vision and values of AHS.
Completion of an accredited practical nurse education program. Active or eligible for registration and practice permit with the College of Licensed Practical Nurses of Alberta (CLPNA). Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP).
Minimum of two years' experience within the last five years of Home Care experience, although skills and experience relevant to nursing practice in community sector will be considered. An excellent attendance record, reliability and personal suitability must be demonstrated through your employment references. Physically able to perform all assigned tasks and may involve physical exertion not limited to climbing stairs, walking outside, lifting, pulling, pushing, stooping, and carrying which may involve visits to client homes that are not ideal.
Experience in providing case management services with knowledge o Connect Care and service authorizations. Experience in Home Parenteral Therapy (HPT), wounds, lower limb assessments, skin and ostomy care, palliative support and client teaching. Aware of Continuing Care Health Service Standards and the ability to incorporate these standards in your practice.
