Description
Vermilion Long Term Care is a 48 private bed, cottage style facility built in 2007. Each room is equipped with a full lift track system, private bathroom and shower and spacious living area. As an LPN, you are a key part of an interprofessional health care team and will provide professional nursing services to individuals who have complex, unpredictable medical needs and require onsite 24 hours assessment and treatment. RN onsite 24 hours a day.
As a Licensed Practical Nurse (LPN), you are part of an interprofessional health care team utilizing nursing processes, critical thinking, problem solving and decision making skills. You will be guided by your education, experience, demonstrated skill level and AHS policies and procedures. You will play a key role in providing safe, quality patient and family centered care while reflecting the shared vision and values of AHS.
Completion of an accredited practical nurse education program. Active or eligible for registration and practice permit with the College of Licensed Practical Nurses of Alberta (CLPNA). Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP).
Knowledge of Medication Assistance Program (MAP). Ability to support the HCAs who participate in this program. Strong communication and leadership skills. The ability to balance numerous demands and conflicting priorities is essential. The candidate must be able to work well independently and within a team. Demonstrates critical thinking skills, including clinical judgment, problem solving skills and strong decision-making ability. Required to work to full scope of practice of LPN.
Connect Care - Epic - Continuing Care Facility Living Nurse and EUPA. Continuing Care Health Services Standards Education Completion. Familiar with Admission and Discharge Processes in Continuing Care Facilities. Familiarity with Interfacility Transfer Processes in Connect Care. Have 1 to 2 years recent experience in LTC/ Supportive living / Seniors Health. Understanding of collaborative care and resident focus care. Dementia Care Training. Experience working with Palliative Care.
