Description
To work with and lead the daily clinical operations of our palliative care consult team to provide quality palliative and end of life care to patients and families within the Calgary zone. Our physicians, clinical nurse specialists, nurse practitioners, palliative consultants and administrative support staff are compassionate, committed professionals who function in small consult teams supporting patients, families, and teams across all locations of care within the Calgary zone.
The Care Manager of the Palliative Care Consult Service - Calgary Zone, performs a leadership role reporting directly to the Area Manager Palliative Care Consult Service/Hospice Operations/Palliative cARP Manager. The care manager is responsible and accountable for daily management of staff and activities associated with the Palliative Care Consult service across all urban and rural care sectors of the Calgary Zone (Acute Care, Community - Home, Long Term Care and Supportive Living). The care manager works with the team of staff comprised of Nurse Practitioners, Clinical Nurse Specialists, Clinical Instructors, Palliative Care consultants and Administrative Support staff to provide effective and efficient delivery of quality patient and family centered care throughout the zone. In collaboration with the Area Manager, the care manager is accountable for hiring, professional development and performance management of the staff. Collaborating with our palliative consult physician partners and community partners, this position is also responsible to provide team leadership, scheduling staff according to service needs, educational leadership, contributing to program improvement, strategic planning, change leadership and risk management. This position promotes an environment that fosters quality improvement and a culture of safety for staff and patients and commits to continually improving health and safety performance through hazard assessment, risk management, incident identification, reporting and correction, and compliance with applicable regulations, policies, and safe work practices. The Care Manager may provide coverage for other leadership within the program as needed.
Health Care related Baccalaureate Degree required in addition to membership (in good standing) with an accredited health care professional association. A minimum of 5 years in a professional/clinical Health Care role ideally in an organization of significant size, complexity and diversity; 3 years of leadership experience such as a charge nurse, clinical nurse educator, team lead or clinical project lead required.
Familiar with Palliative/End of Life Care principles and practices. A positive and enthusiastic attitude is required, and a willingness to actively tackle new challenges, an ability to communicate in a professional, compassionate and effective manner. Ability to foster a climate of collaboration and cooperation; foster partnerships to achieve organizational goals across all care sectors within the Calgary Zone with experience managing staff within a unionized environment; build solid relationships with partners in care, committees and other internal and external key stakeholders. Strong organizational skills, influential skills, patient and family centered focused orientation and commitment to providing quality services within a team environment. Leadership skills, including demonstrated ability and comfort with decision making responsibilities, change management, coaching and teaching, conflict resolution and the ability to inspire and build confidence in others as individuals and within teams. Ability to manage human, financial and physical resources within an operating environment. Ability to integrate and apply clinical excellence in the daily management of the area; and ability to manage change and create solutions for complex and diverse issues.
Previous management experience in health-related field preferred. A Masters degree in Business Administration, Health Care Leadership, Health Care Administration or a health-related field is an asset. Understanding of the advanced practice nurse role is an asset.
