The transition units meet the need to move clients from acute care to a place for rehabilitation prior to discharge to the most appropriate living situation. There are six units in three facilities. There are three specialized units, neuro-rehab, musculoskeletal and an enhanced care unit (medically complex). The Regional Community Transition Program provide a multidisciplinary approach to rehabilitation for clients following surgery, stroke, lengthy hospitalizations, or other physical/medical issues requiring further rehabilitation prior to clients returning home. The units are covered by Mental Health Clinicians and Psychiatrists. Transition referrals cover a wide variety of referral reasons including adjustment, anxiety, depression, schizophrenia, bi-polar disorder delirium, and dementia, to name a few. The team is also referred for determining capacity and enacting legal documents (Personal Directive/Enduring Power of Attorney). It is often the first time a client is seen and assessed for cognitive decline, and this may or may not impact where the person is discharged to live post rehab. This position has been identified and is anticipated to transition to Recovery Alberta effective September 1, 2024. As a Mental Health Consultant, you will work as a member of an interdisciplinary team responsible for providing assessment and treatment recommendations to professional caregivers and family physicians regarding the management and treatment of individuals with complex mental health disorders. You will also be responsible for the provision of education and support to professional caregivers and family members, and for consultation and liaising with other professionals along a continuum of care to facilitate a coordinated network of mental health services.

Completion of one of the following programs: Master's degree in Psychology. Master's degree in Social Work (clinical specialization) including education in mental health assessment and psychotherapy. Master's or Bachelor's degree in Occupational Therapy including training and experience in mental health assessment and provision of psychotherapy. Master's degree in Nursing or Psychiatric Nursing including training and experience in mental health assessment and provision of psychotherapy. Active or eligible for registration with the applicable regulatory college.

Minimum 3 years mental health experience, which includes at least 1 year working in geriatric mental health. Experience working on a multidisciplinary team and in collaboration with other health partner agencies. Experienced in assessment and report writing. Ability to read lab values. Experience working in a consultation role. Extensive knowledge of clinical mental health syndromes and medical issues commonly seen in the senior’s population. Proficient understanding of dementia and delirium syndromes as well as treatment practices for this subgroup of mental health clients. Ability to work in a fast-paced subacute environment. Ability to work independently and within an outreach model. Strong mental health and medical assessments, consultation and intervention skills. Excellent verbal and written communications skills essential. Excellent interpersonal skills required. Basic computer and word processing skills required. Ability to set priorities and organize time effectively. Current BCLS-HCP certification required.
