Location may be negotiable. The Palliative Care Nurse Practitioner (PCNP) is a self-directed practitioner who directs, provides, and evaluates patient care based on advanced practice nursing and medical management knowledge and skills. The PCNP has a greater breadth, depth and complexity of application of nursing knowledge, skills, attitudes, judgement and performs additional activities beyond the Registered Nursing practice (CRNA, 2011). This position functions as an integral member of the Palliative/End of Life Care portfolio by combining the roles of practitioner, teacher, consultant, leader, and researcher through integrating inter-disciplinary clinical practice, nursing staff development and research activities. The PCNP helps to improve frontline nursing practice and promote advanced practice nursing within the palliative care nursing consultation team. The majority of this role is spent in the provision of timely and effective clinical consultative care provided to the palliative care patient population in a variety of settings including community, rural, continuing care and acute care. The PCNP provides direct mentoring to improve palliative care expertise within the inter-disciplinary healthcare team. The PCNP will be leading advancement of palliative interdisciplinary clinical practice including developing, implementing and evaluating education programs, teaching, leading or participating in working groups, and participating in research projects related to Palliative Care. This position requires travel within the Central Zone.
As a Nurse Practitioner (NP), you will provide a full range of comprehensive health services to individuals across their lifespan. You will be responsible for integrating clinical skills associated with nursing and medicine to assess, diagnose, order, and interpret diagnostic tests, prescribe treatment, and perform procedures to manage and meet the health needs of patients. Additionally, you will utilize advanced practice nursing competencies related to Clinical Practice, Leadership and Optimizing Health Systems, Education, and Research to increase care quality, promote appropriate use of health services, and inform and influence healthcare systems. Successful completion of an approved Nurse Practitioner program and a minimum of master's degree. Registration with CRNA on the Nurse Practitioner Registrar including Provisional Graduate Nurse Practitioner licensure. Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP).
Minimum of 5 years of clinical experience as a NP. Minimum 1 year of clinical experience managing palliative care clients (within the last 3 years) in an advance practice role. Minimum 1 year of Seniors Health experience. Experience in at least two (2) care sectors, including Home Care. Advanced clinical & assessment skills.
CNA Certification in CHPCN(C) ®Certified in Hospice Palliative Care Nursing Canada. Case Management Education. Meditech and Connect Care trained. Experience in program planning, education of adults, interdisciplinary practice, and research activities. Excellent interpersonal & communication skills. Ability to work independently & to be flexible to adapt to changing clinical situations. Works collaboratively with all members of the health care team, including patients and their families.
