The Alberta Kidney Care-South Nurse Practitioner (NP) is a self-directed advanced practice nurse who functions in a collaborative practice model. The Alberta Kidney Care-South NP demonstrates comprehensive knowledge in the area of medicine and nephrology as well as the broader diagnosis and differential diagnoses, advanced assessment skills and treatment surrounding complex patients within the Nephrology teams. The NP performs restricted activities within their scope of practice as defined by Schedule 24 of the Health Professions Act, the Registered Nurses Profession Regulations and the College and Association of Registered Nurses of Alberta (CARNA). Nursing activities performed are determined by legislation, regulation, regulatory body standards and guidelines, Alberta Health Services (AHS) policy, educational preparation, practice setting, patient need and competency of the individual NP. Providing timely access to care is a key function of the Alberta Kidney Care-South NP role. Care is provided through knowledge in advanced practice assessment skills, pathophysiology, and pharmacotherapeutics. These key capabilities allow for the development of differential diagnoses and indication for further interventions. The NP will help provide continuity of care to more complex patients and provides continuity as part of the interdisciplinary team to patients and their families. This will include complex care planning across different points of transition from newly diagnosed, through chronic care management to complex discharges to acute care admissions. NP is a critical role to pull healthcare system together as a team across disciplines in order to provide comprehensive, seamless and connected care across teams, sites, and disciplines.

As a Nurse Practitioner (NP), you will provide a full range of comprehensive health services to individuals across their lifespan. You will be responsible for integrating clinical skills associated with nursing and medicine to assess, diagnose, order, and interpret diagnostic tests, prescribe treatment, and perform procedures to manage and meet the health needs of patients. Additionally, you will utilize advanced practice nursing competencies related to Clinical Practice, Leadership and Optimizing Health Systems, Education, and Research to increase care quality, promote appropriate use of health services, and inform and influence healthcare systems. Successful completion of an approved Nurse Practitioner program, with a minimum of a Master's degree. Active registration with the College of Registered Nurses of Alberta (CRNA) on the Nurse Practitioner Registrar, including Provisional Graduate Nurse Practitioner licensure. Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP).

Follow the CRNA Scope of Practice for Nurse Practitioners, Nurse Practitioner Competencies and Nursing Practice Standards in conjunction with other applicable CARNA documents and Canadian Nurses Association Code of Ethics. Adhere to organizational policies, directives, protocols and procedures; question if they are not current and/or evidence informed. Manage resources in a fiscally responsible manner within areas of responsibility. Demonstrated ability to function effectively both independently and as a member of an interdisciplinary team. Demonstrates excellent interpersonal and communication skills when interacting with other care providers, patients and their family members. Demonstrated skills in leadership including mentorship, coaching, quality improvement, strategic thinking, program development. Demonstrated strong Computer literacy skills (i.e. Connect Care, etc.). Adult Medicine and/or Nephrology experience in an advanced practice role. Experience working with acute care adult medicine and/or nephrology hematology patients within the last 5 years; Ambulatory and/or Inpatient Acute Care. Experience with the development and/or implementation of evidence-based practice. Experience with program development, change management, quality improvement. Completion of leadership related courses; within the last 3 years. Progressive nursing leadership experience within the last 5 years.

CNeph(C) Certification an asset. Experience as a RN/NP in nephrology within the last 5 years an asset. Experience as a RN/RN in a chronic disease management setting within the last 5 years an asset. Experience in Case Management process of care within the last 5 years an asset. Experience with management of diabetes, hypertension and cholesterol an asset. Experience with patient education/application of the principles of adult learning. Demonstrated electronic health record documentation skills.
