Description
AHS is dedicated to enhancing care in the community, helping people/families be healthy, well and independent in their homes and communities. Edmonton Zone Continuing Care provides services through five main program areas: Home Living, Supportive Living, Facility Living, Transition Services and Specialty Programs. The Specialty Programs portfolio includes Children’s Home Care (CHC). CHC is a multidisciplinary team with RN, SW, RT, PT, OT, SLP and TAs collaborating and delivering services to children from birth - 18 years old with a wide variety of medical and/or developmental needs/conditions, including palliative care. The CHC program serves the Edmonton Zone, and frequently collaborates with suburban/rural Home Living partners to provide quality care. CHC has an open Occupational Therapist Team Lead (OT II) position. As an OT II for CHC, you are an advocate, mentor and coach for your colleagues, working closely with leadership to identify emerging practice for program implementation. You are also a Case Manager (CM), & a direct care provider. Case manager and direct provider duties include using a family centered approach when completing ongoing assessments, identification of the unmet needs of the child/family, & evaluation of outcomes. In collaboration with the client/family and other partners, using appropriate resources, the CM develops a care plan for supports and services that is modified, based on the client’s response to the interventions.
As an Occupational Therapist II, you will be responsible for the coordination of occupational therapy and other allied health clinical services within a program or service area. You will provide supervision of students and team members as assigned. As an Occupational Therapist II, you will be responsible to work both independently and as part of an interdisciplinary team where you will be an integral member, providing leadership to other healthcare professionals. You will be making day-to-day clinical decisions on patient/family needs regarding assessment, intervention, discharge and follow-up. You will make decisions and advise team members on complex clinical cases and provide clinical leadership within the program or service area to ensure safe, efficient and effective client care. You will ensure occupational therapy practice is evidence-based and in alignment with Alberta legislation and regulations, professional standards of practice, code of ethics and AHS policy and guidelines. You will be providing coordination of the day-to-day operations of an assigned service area and monitoring care delivery, identifying service needs and assisting other medical staff with caseload management, completing chart audits and providing follow-up and feedback to staff. You will be participating in program development and operational planning and representing the department on committees or programs. You will be making decisions on staffing level requirements, workflow and caseload priorities in collaboration with managers and other healthcare providers. As assigned, you will carry out the roles of a direct clinical service provider, consultant, educator and/or advocate in your program or service area.
Master's degree in Occupational Therapy from an accredited university program or bachelor's degree in Occupational Therapy from an accredited university program and related experience. Active or eligible for registration and practice permit with the Alberta College of Occupational Therapists (ACOT).
Knowledge of developmental milestones and ability to implement appropriate strategies for treatment in the following: Feeding & swallowing for children at increased risk of aspiration. fine motor development sensory issues, cognitive development, positioning and muscle tone, visual and/or hearing impairment, and neurodevelopment. Assistive technology. Able to bend, lift and carry medical bags and/or equipment, weighing approximately 12-20kg. Must be able to navigate various computer programs including Microsoft Office, Outlook, etc. At the time of interview, you may be required to demonstrate your computer knowledge and proficiency by completing a hands-on skill test. Can mentor staff in direct clinical skill development, case management, and electronic documentation. 4 years current experience with pediatric clients. Experience using the principles of adult learning to conduct education sessions, in-services and program development implementation and evaluation is required.
Experience with Connect Care. Demonstrated ability to work as part of an interdisciplinary team. Excellent interpersonal and communication skills. AADL Authorizations with knowledge of pediatric specific equipment and environmental adaptations. McMaster Case Management training and understanding of the AHS Continuing Care Case Management Competency Framework. Excellent organizational and time management skills.
