This position reports directly to the Postpartum Community Services (PPCS) Care Manager and Area Manager, Public Health, Calgary Zone. This incumbent will work out of the East Calgary Health Center. The Outreach Worker (OW) is a key member of the Postpartum care team, collaborating closely with the Postpartum Social Worker (SW), and Public Health Nurse to assist families in meeting needs relevant to the social determinants of health. Under the direction of the SW, the Outreach Worker assists primary caregivers and families with vulnerabilities by providing informal support, referrals to community agencies for basic needs (such as food or finance) and assists parents in completing a variety of forms/applications such as the AB Online Birth Registration form and Refugee Claimant applications. The Outreach Worker provides key information and resources required for client financial and social support as well as health care access. This position works with many diverse cultures and clients and engages interpretation services as required, using the AHS Language Line. Strong organizational and time management skills and the ability to prioritize and make independent decisions, communicate effectively in person and on the phone are all required of this position. Employee is expected to work collaboratively, as a team player, within a culturally diverse, multidisciplinary team. Employee must demonstrate strong work ethic, accountability to follow program guidelines, processes and expectations in accordance with the Health Information Act and Freedom of Information and Privacy and maintain a professional demeanour in accordance with the AHS values of Compassion, Accountability, Respect, Engagement and Safety.

As an Outreach Worker, you will work as a member of a multidisciplinary team, supporting clients to stabilize in their current environments as an extension of their clinical care service. As an Outreach Worker, you will conduct outreach assessments, determine outreach service goals and develop outreach treatment plans in collaboration with clients and clinicians. You will facilitate client connections with community and government supports and advocate for clients with these external service providers. You will focus on establishing housing, financial, food and clothing, transportation and social supports for clients. You will be responsible for providing education and coaching support related to daily living skills and activities (attending appointments, money management, searching for jobs, grocery shopping, etc.), observing and reporting changes in the clients' behaviours, participation and ability to function independently to the interdisciplinary team. You will evaluate client progress and functioning and make adjustments to the outreach treatment plan as required. As a part of your role as an Outreach Worker, you may be required to provide outreach crisis stabilization support for clients, exercising various de-escalation techniques and linking clients with the appropriate supports (EMS, Police and Crisis Team, etc.). You also may be required to enter clients' residences and assist clients to transfer in and out of vehicles, care facilities and residences.

Completion of diploma from a post-secondary program in a related field.

Post-secondary education, college diploma or university degree in health, education or a human services field (Social services diploma, SW diploma, Human Services or Family/Child Studies diploma). Minimum 2 years' experience working as an Outreach worker in a community setting. Experience with City of Calgary community resources. Proficient computer skills. Strong organizational and time management skills. Strong written and verbal communication in the English language. Must be physically able to lift/carry/pull/push up to 25lbs; sit for long periods at a computer, as well as move as needed throughout the office setting.

Degree in the Human Services Field (Social Work, Psychology). Knowledge of City of Calgary Postpartum Parenting/Family resources. Connect Care. Fluency in other languages.
