The Chronic Disease Management (CDM) Cardiac Nurse utilizes the nursing process, critical thinking and problem-solving skills to provide care to clients across the lifespan in the area of chronic disease management. The CDM Cardiac Nurse ensures the development, implementation and ongoing administration of the client care plans. The CDM Cardiac Nurse assesses, coordinates, supervises, administers and evaluates the provision of health services and healthcare resources. The CDM Cardiac Nurse accepts responsibility for continuing competencies through ongoing professional development, including participation in education programs, research and continuous quality activities. The CDM Cardiac Nurse demonstrates the ability to accurately and effectively assess client status and to take appropriate intervention measures that have a significant impact on the client's medical condition, safety, as well as ultimate recovery. The CDM Cardiac Nurse may provide education and support in the areas of diabetes, weight management, cardiac risk reduction, secondary stroke prevention, and lifestyle management. In addition to a competitive rate of pay, AHS currently has a Remote Retention Allowance in place to an annual maximum of $3,000. This allowance is non-pensionable and is payable on an hourly basis for all hours paid at the basic rate of pay.

As a Registered Nurse (RN), you will provide a wide variety of nursing services to patients, families, communities and populations, while taking necessary steps to ensure their safety and well-being. In your role, you will utilize nursing processes, through critical thinking, problem solving and decision making, as well as teach, counsel and advocate on behalf of patients and their families. You will play a leadership role in supporting an integrated and holistic approach to patient care, health promotion and maintenance. You will provide safe, quality patient and family centered care while reflecting the shared vision and values of AHS. PLEASE NOTE: Provisional Permit Holders shall be paid $35.87/hour - $43.82/hour. Upon being entered into the register of Registered Nurses, the rate of pay shall be adjusted to the rate of pay applicable to a Registered Nurse in accordance with AHS/UNA Article 25.02(b)(i) or (ii) as applicable. Completion of an accredited nursing education program. Active or eligible for registration and practice permit with the College of Registered Nurses of Alberta (CRNA). Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP). If you have not practiced as a Registered Nurse within the last five (5) years, completion of the following free courses is required: CRNA Medication Management - Chapters 1 - 4 (https://connect.nurses.ab.ca/home/%2Flearning-and-development%2Flearning-modules%2F) and the NextGenU Health Assessment Resources Certification (https://courses.nextgenu.org/course/view.php?id=271#section-0)

Experience in cardiac risk reduction, diabetes education, weight management, and lifestyle/behaviour change across the lifespan. Experience in cardiac rehabilitation. Experience in facilitating chronic disease management education sessions, utilizing health change and motivational interviewing skills, and supporting clients in both group and individual appointment settings. Community engagement experiences are essential. Alberta Health Services hires new graduate nurses with the required educational requirements as above. Physical Effort/Strength required: Physical demands include lifting, pulling and pushing, walking, standing, transferring and transporting clients and equipment frequently.

At least one year of relevant nursing experience is desired.
