This position reports to the Home Care Manager, in Fort McMurray, Alberta. This position provides Home Care services located within the Regional Municipality of Wood Buffalo and outlying communities. The Home Care Registered Nurse Case Manager provides case management services and interventions to clients with a variety of healthcare needs. As a case manager, you will be responsible for the oversite and coordination of the healthcare needs of clients across the lifespan, including chronic disease management, health assessments, assessment for assistance with ADL’s, wound assessments, wound care, assessments for placement in DLO facilities as well as palliative and end of life care. AHS respects and supports client choices and collaborates with individuals, caregivers, professionals, and other community resources to optimize healthy living and connectedness within the community. The Home Care Registered Nurse Case Manager is responsible for the assessment, planning, implementation, and evaluation of client care, in the setting most appropriate for the client. The incumbent performs a variety of nursing duties in accordance with clinical policies and nursing care standards. Note: The ability to drive in long distances in all weather conditions. In addition to a competitive rate of pay, AHS currently has a Fort McMurray Allowance in place to an annual maximum of $12,480. This allowance is non-pensionable and is payable on an hourly basis for all hours paid at the basic rate of pay.

As a Registered Nurse (RN), you will provide a wide variety of nursing services to patients, families, communities and populations, while taking necessary steps to ensure their safety and well-being. In your role, you will utilize nursing processes, through critical thinking, problem solving and decision making, as well as teach, counsel and advocate on behalf of patients and their families. You will play a leadership role in supporting an integrated and holistic approach to patient care, health promotion and maintenance. You will provide safe, quality patient and family centered care while reflecting the shared vision and values of AHS. PLEASE NOTE: Provisional Permit Holders shall be paid $35.87/hour - $43.82/hour. Upon being entered into the register of Registered Nurses, the rate of pay shall be adjusted to the rate of pay applicable to a Registered Nurse in accordance with AHS/UNA Article 25.02(b)(i) or (ii) as applicable. Completion of an accredited nursing education program. Active or eligible for registration and practice permit with the College of Registered Nurses of Alberta (CRNA). Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP). Minimum of 3-5 years recent nursing experience. Computer skills well developed in Meditech Care Manager, Microsoft office, Outlook. Expertise in wound care, wound vac, continence care and ostomy care. Ability to prioritize and manage time effectively. Demonstrated ability to show initiative and work independently. Utilizes expert nursing knowledge, complex decision-making and critical thinking skills, collaborative approaches to care, evidence informed practice, and strong nursing competencies. Uses advanced knowledge, skills, and judgment to improve client care and manage complex and challenging situations. Evidence of excellent verbal and written communication skills. Adherence to the CNA professional Code of Ethics, Registered Nursing Competencies. Ability to meet the physical demands of the job.

A Master’s Degree in nursing or a related field is preferred. Aids to daily living (AADL) authorization experience preferred. 1-3 years of Home Care Case Management experience. Medical Surgical experience an asset.
