Suppose you are interested in working within an inter-professional team playing a key role in linking the needs of Rural Home Care clients with appropriate health and community services. In that case, this Registered Nurse position will be of interest to you. Based on the Continuing Care Case Management Competency Framework you demonstrate ability in personal and professional accountability, systems thinking, facilitating creative thinking, communicating and building relationships. Using a person-centred approach, you will develop a care plan supporting the client/family to identify needs, focus on strengths, set achievable personal health and self-care goals and provide direct nursing care to individuals across the life span. In collaboration with the client/family, you will ensure services are provided and modified on an ongoing basis, based on the client’s response to interventions, incorporating stewardship into service authorization. As a Benefits Eligible Casual Employee with a guaranteed specified FTE of no less than 0.4 FTE and no specified hours per Shift or Shifts per Shift cycle. Please note that this position is 131 km south outside of the Calgary city limits and approximately 84 km NW outside of the Lethbridge city limits.

As a Registered Nurse (RN), you will provide a wide variety of nursing services to patients, families, communities and populations, while taking necessary steps to ensure their safety and well-being. In your role, you will utilize nursing processes, through critical thinking, problem solving and decision making, as well as teach, counsel and advocate on behalf of patients and their families. You will play a leadership role in supporting an integrated and holistic approach to patient care, health promotion and maintenance. You will provide safe, quality patient and family centered care while reflecting the shared vision and values of AHS. PLEASE NOTE: Provisional Permit Holders shall be paid $35.87/hour - $43.82/hour. Upon being entered into the register of Registered Nurses, the rate of pay shall be adjusted to the rate of pay applicable to a Registered Nurse in accordance with AHS/UNA Article 25.02(b)(i) or (ii) as applicable. Completion of an accredited nursing education program. Active or eligible for registration and practice permit with the College of Registered Nurses of Alberta (CRNA). Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP). If you have not practiced as a Registered Nurse within the last five (5) years, completion of the following free courses is required: CRNA Medication Management - Chapters 1 - 4 (https://connect.nurses.ab.ca/home/%2Flearning-and-development%2Flearning-modules%2F) and the NextGenU Health Assessment Resources Certification (https://courses.nextgenu.org/course/view.php?id=271#section-0)

Handling, moving and lifting heavy or awkward equipment and/or individuals are required. Ability to perform all duties following related legislation. Intermediate level of computer skills. Excellent organizational and time management skills with strong interpersonal skills.

A baccalaureate degree in nursing is preferred; however, skills and experience relevant to nursing practice in the community sector will be considered. Minimum of two years of current nursing experience with one year in a community-based setting preferred. Recent experience working with acute clients and/or long-term clients with a focus on chronic disease management. Home Care experience preferred.
