Description
The Adult Community Services portfolio is recruiting to this exciting opportunity which will assist in supported discharge for clients from acute care to the Addiction and Mental Health - Adult Community Services programs. Reporting to the Care Manager and under the direction of the Program Manager, the Transition Coordinator functions in the context of a multi-disciplinary team that provides quality, client-focused, recovery-based, concurrent capable, culturally appropriate care to those seeking assistance/services within Addictions and Mental Health. The Transition Coordinator is responsible for facilitating timely transitions for those clients who require supported discharge from acute care to one of the Adult Community Services programs. The Transition Coordinator will liaise with the acute care and act on behalf of the community team to ensure a coordinated effort to ensure continuity of care. The candidate will work with the multidisciplinary team to identify and address any barriers that may occur regarding discharge. Additionally, the Transition Coordinator will work with existing Adult Community Service clients to assist with connections to Primary Care as needed. This position has transitioned to Recovery Alberta effective September 1, 2024. As a Social Worker II, you will be a member of an inter-disciplinary team and provides a range of social work services to patients and their families. Your services may include: psychosocial assessment, resource-based interventions, individual, group and family treatment, risk screening, counselling, case management and coordination, transition or discharge planning, and prevention, promotion and public awareness activities.
Completion of bachelor's degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW).
Minimum of 3 years of relevant mental health experience, preferably in a community setting. Must have a strong knowledge of community resources and provincial legislation such as Mental Health Act and Community Treatment orders. Must have current BCLS-HCP certification and non-violent crises intervention. Conflict resolution and strong communication skills are required. Must be able to work independently and within a team environment. Ability to problem solve and demonstrate critical thinking skills is required. Ability to ambulate, climb stairs, and move quickly without limitations in a variety of environments.
A minimum of five years of acute care social work experience is preferred.
