We have an exciting opportunity for a Social Worker II who will have opportunity to work within a dynamic environment that is designed to meet the needs of individuals who present with a combination of dementia syndrome and complex behavioral issues. Residents may exhibit challenging, unpredictable responsive behaviors which require enhanced staffing expertise to provide care and oversight. Reporting to the Area Manager Allied Health, the Social Worker II collaborates with the Care Community to assess, plan, implement, and evaluate care. The Social Worker II will be part of a dynamic interdisciplinary team with a strong focus on the multi-disciplinary and collaborative process, including and focused on the patient. These services are primarily clinical in nature, and require expertise in financial and resource management, psychosocial assessments, counseling, case management and transition planning. We have Therapy Assistants, Occupational Therapists, Speech Language Pathology, Recreation Therapy and Social Work on our Allied Health team working in close collaboration with our nursing teams. In addition to a competitive rate of pay, AHS currently has a Remote Retention Allowance in place to an annual maximum of $3,000. This allowance is non-pensionable and is payable on an hourly basis for all hours paid at the basic rate of pay.

As a Social Worker II, you will be a member of an inter-disciplinary team and provides a range of social work services to patients and their families. Your services may include: psychosocial assessment, resource-based interventions, individual, group and family treatment, risk screening, counselling, case management and coordination, transition or discharge planning, and prevention, promotion and public awareness activities.

Completion of bachelor's degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW).

Assessment skills to recognize and respond appropriately to changes in client status, and develop appropriate care plan.

Clinical experience as a medical Social Worker, as well as specific experience in the management of Dementia Syndrome will be preferred.
