Reporting to program manager of NW Campus, the Social Worker II provides a range of social work services to patients and their families on Unit 24 outpatient mental health unit. Working within the context of patient and family-centered care and multidisciplinary service delivery, the successful applicant will collaborate with a range of other service providers within AHS and the community to provide support and intervention to patients on an outpatient mental health unit setting. The focus of this position will be working collaboratively with the case managers, psychiatrist and therapist to support and mitigate the social needs of our patient population. This position has been identified and is anticipated to transition to Recovery Alberta effective September 1, 2024. As a Social Worker II, you will be a member of an inter-disciplinary team and provides a range of social work services to patients and their families. Your services may include: psychosocial assessment, resource-based interventions, individual, group and family treatment, risk screening, counselling, case management and coordination, transition or discharge planning, and prevention, promotion and public awareness activities.

Completion of bachelor's degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW).

Demonstrated ability to collaborate with other service providers and to adapt practice to the unique needs of a range of patients, their families, and community service providers required. Excellent knowledge and understanding of the Assured Income for the Severely Handicapped (AISH) program, Employment Insurance (EI) program, social services and how to assist patients in accessing these programs as required. Strong mental health assessment and crisis management skills required. Solid knowledge of intervention and treatment modalities relevant to clinical disorders and psychosocial and environmental problems required. Comprehensive knowledge of relevant community resources and legislation required. Experience in discharge planning for patients transitioning back into the community resources required. Skilled in knowing how to engage and disengage from therapeutic alliances required. Understanding of the mental health continuum of Care required. Understanding of Recovery model required.

Previous experience in an outpatient setting. Master’s degree preferred. Minimum 2 years Social Worker experience providing clinical services to addictions and mental health clients preferred.
