The Peter Lougheed Centre (PLC) Addictions & Mental health Program is a group of dynamic multidisciplinary teams that work together to ensure our patients have the best standard of care & experience. Our site runs 6 Addictions & Mental Health Programs; 2 General Psychiatry Inpatient Units, Day Hospital Program, Psychiatric Emergency Services (PES), Consultation Liaison, & the Short Stay Unit (SSU). You have a pivotal role as an integral member of our multidisciplinary team. Focused on in-patient acute psychiatric care, you will engage in collaborative efforts to provide holistic & comprehensive care, addressing the diverse needs of individuals, families, & communities. You will work collaboratively with the multidisciplinary team to develop a robust plan of care to stabilize patients suffering from acute psychiatric illness. Your responsibilities include delivering targeted interventions, with an awareness of evaluation to modify approaches based on evolving patient needs. Your practice will encompass holistic mental health assessments, de-escalation, & conflict-resolution. You will engage in discharge planning that promotes, maintains, & restores mental health, while preventing mental illness injury, & disability. You will be responsible for facilitating referrals to community providers, in addition to providing community resource recommendations to the care team. This role also engages in the delivery of the Mental Health Act (MHA) in coordinating Community Treatment Orders (CTO’s). You will engage in the delivery of the Adult Guardianship & Trusteeship Act (AGTA) through the administration of guardianship applications & capacity assessment referrals. As a Social Worker II, you will be a member of an inter-disciplinary team and provides a range of social work services to patients and their families. Your services may include: psychosocial assessment, resource-based interventions, individual, group and family treatment, risk screening, counselling, case management and coordination, transition or discharge planning, and prevention, promotion and public awareness activities.

Completion of bachelor's degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW).

Basic Cardiac Support- Health Care Provider (BCLS-HCP) certification required. Demonstrated ability to collaborate with other service providers and to adapt practice to the unique needs of a range of patients, their families and community service providers required. Excellent knowledge and understanding of the Assured Income for the Severely Handicapped (AISH) program. Employment Insurance (EI) program, social service and how to assist patients in accessing these programs as required. Strong mental health assessment and crisis management skills required. Solid knowledge of intervention and treatment modalities relevant to clinical disorders, psychosocial and environmental problems required. Comprehensive knowledge of DSM V, Alberta Mental Health Act (MHA), Adult Guardianship & Trusteeship Act, and Personal Directives Act required. Experience in completing referrals to Addictions and Mental Health community programs, including the completion of Community Treatment Orders (CTO’s) under the MHA required. Discharge planning for patients transitioning back into the community required. Training in NVCI required.

Minimum 2 years Social Worker experience providing clinical services to addictions and mental health clients preferred. Master's degree preferred. Knowledge of housing resources an asset.
