Located in N.E. Calgary, the Peter Lougheed Centre (PLC) is an acute care hospital offering both inpatient and outpatient services. The PLC has over 600 inpatient beds, including Medical, Surgical, Pediatrics, Women’s Health and Critical Care. The PLC is the Vascular Centre for Southern Alberta, Southwestern Saskatchewan and Southeastern British Columbia. The Allied Health Department at the PLC is comprised of Social Work, Rehabilitation, and Spiritual Care staff. These staff work as vital members of the interdisciplinary team to provide patient and family-centered care throughout the hospital. Under general supervision of the Clinical Leader and Allied Health management, the successful candidate will provide Social Work services to medical and surgical units at the Peter Lougheed Centre (PLC). These services focus on direct clinical care and encompass psychosocial assessments, counseling, case management, and discharge planning. Caseloads may vary to meet operational needs, and additional support to other patient care units, including the emergency department, may be required to ensure balanced workload distribution within the Social Work team. This role demands both independence in a fast-paced environment and strong collaboration within an interdisciplinary team. The Social Worker will maintain strict adherence to the Alberta College of Social Workers (ACSW) standards of practice to ensure high-quality, ethical care at all times.

As a Social Worker II, you will be a member of an inter-disciplinary team and provides a range of social work services to patients and their families. Your services may include: psychosocial assessment, resource-based interventions, individual, group and family treatment, risk screening, counselling, case management and coordination, transition or discharge planning, and prevention, promotion and public awareness activities.

Completion of bachelor's degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW).

Relevant experience working in a large, urban adult acute care setting within the last 3 years. Recent Social work experience within an adult acute care setting on medical and surgical units. Recent experience working with patients/families facing chronic illness. Strong understanding of trauma informed care principles. Basic Cardiac Life Support - Health Care Provider (BCLS-HCP) certification required. Excellent interpersonal and communication skills to interact with members of the interdisciplinary team, patients and families in a dynamic health care setting. Demonstrated problem solving, critical thinking and conflict resolution skills. Excellent organizational and time management skills to efficiently organize and prioritize workload to keep pace in a busy multi-disciplinary patient care. Demonstrates behaviours consistent with the values of AHS - Compassion, Accountability, Respect, Excellence, and Safety. Experience working with diverse communities, groups and/or organizations (i.e. ethno-cultural, equity-setting, racialized, faith-based, LGBTQ2S+, vulnerable, marginalized, persons with disabilities, and persons across all age groups, etc.).

Recent Social work experience within an acute care setting. Recent experience working with patients/families facing chronic illness. Experience working in Emergency dept and Mat/Child unit is an asset. Previous Connect Care Training an asset.
