This is an exciting opportunity for a Social worker!! in the Adult Inpatient Program at the Centennial Centre for Mental Health and Brain Injury (CCMHBI) in Ponoka. CCMHBI is a campus of specialists and health professionals that are committed to integrated, multi-disciplinary care and treatment to ensure the best outcomes for patients and their families. The Social Worker II position is based in the Paragon Unit and requires a highly motivated individual that is keenly interested in working with patients within an acute psychiatric setting. The Acute Services Units within the Adult Inpatient Program at CCMHBI provides comprehensive assessment, treatment, and stabilization services to individuals 18-64 years of age who have a severe and persistent mental illness and/or are experiencing an acute mental health crisis and/or addiction illness. Strong interpersonal, organizational and communications are large contributors to success in this position. You will be an active member of the interdisciplinary team participating in team conferences and care planning. Collaboration with families and communities and comprehensive discharge planning are important aspects of this position. Located in the community of Ponoka, within the parkland region of Central Alberta, the Centennial Centre is one of the most modern Mental Health referral centers in Western Canada. This position has transitioned to Recovery Alberta effective September 1, 2024. As a Social Worker II, you will be a member of an inter-disciplinary team and provides a range of social work services to patients and their families. Your services may include: psychosocial assessment, resource-based interventions, individual, group and family treatment, risk screening, counselling, case management and coordination, transition or discharge planning, and prevention, promotion and public awareness activities.

Completion of bachelor's degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW).
