Description
This Social Worker II position provides quality social work services within the health care setting, in-home care and supportive living sites. This position provides front-line services to clients and families related to the social impacts of their health journey. Assessing social impacts, intervening with supportive counselling, accessing resources, assessing risk, and educating regarding the impacts of legislation and social determinants of health are central. This position requires a broad knowledge of community and government resources, as well as the ability to implement various legislation that guides interactions with adults and seniors. Previous work in a hospital setting and working with seniors on the placement journey would benefit this position. You must be able to work well independently, as well as within the interdisciplinary team. Travel to rural sites within the Zone is a requirement. These include Bassano, Bow Island, and Oyen areas.
As a Social Worker II, you will be a member of an inter-disciplinary team and provides a range of social work services to patients and their families. Your services may include: psychosocial assessment, resource-based interventions, individual, group and family treatment, risk screening, counselling, case management and coordination, transition or discharge planning, and prevention, promotion and public awareness activities.
Completion of bachelor's degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW).
Minimum of three years of professional social work experience is required. Ability to work independently.
Previous experience working in a medical context/setting is preferred. Previous experience working with seniors and their supporters is preferred. Knowledge of community and government resources. Understanding and knowledge of Alternate decision-making legislation. Knowledge of Connect care or EPIC considered an asset.
