The SW II (Navigation) Program is a short-term program implemented to assist with continuity of care and to assist the client to be connected to appropriate community services to support their needs. The program works on a patient centered, goal orientated model with a focus on skills building to promote ongoing independence, reduce distress and improved client outcomes. Early intervention and preventing crises are the cornerstone of this program by promoting increase engagement, additional point of contact, continuity of care and getting the client to the best service possible to address their needs. The SW II will need to be familiar with community agencies and resources, to ensure the client is navigated to appropriate services to meet their identified needs. This could entail finding and familiarizing yourself with community agencies and their programs, organizing and sustaining a database of resources, and continued contact with community agencies/key partners via regular meetings or other communication to remain current. Developing strong working relationships with key partners and agency representatives will be a huge asset in this role both to ensure appropriate referrals to the MH Navigator, and to effectively support the clients’ individual needs in an expedient fashion. This position has transitioned to Recovery Alberta effective September 1, 2024. As a Social Worker II, you will be a member of an inter-disciplinary team and provides a range of social work services to patients and their families. Your services may include: psychosocial assessment, resource-based interventions, individual, group and family treatment, risk screening, counselling, case management and coordination, transition or discharge planning, and prevention, promotion and public awareness activities.

Completion of bachelor's degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW).

2-5 years working in Addiction and mental health.
