The Social Work III role oversees the Dermatology, Rheumatology and General Internal Medicine/Thrombosis clinics at Richmond Road Diagnostic and Treatment Center. This is an advanced clinical position managing a patient population with complex chronic health conditions. In this position the social worker is responsible for; working closely with nursing staff, leadership teams, and other members of the interdisciplinary team, interviewing clients to assess their situation to determine the type of services required, supplying counsel and therapy to help clients resolve social and personal problems, referring clients to agencies that provide financial assistance, legal aid, housing, medical treatment, and other resources as well as planning programs of assistance for clients. Appropriate documentation is required. Opportunity to provide educational in-services to clinical staff.

As a Social Worker III, you will perform either advanced level assessment and treatment or act as a clinical lead. You will work with or within teams to provide health care services for patients with complex and multidimensional medical, psychosocial, environmental and instrumental needs. You will provide advanced clinical services, consultation to the interprofessional team and patients/families and supervision of students and team members as assigned. You may also act as a clinical lead providing leadership in the planning, development, and delivery of services in a specific area, as well as staff development, quality improvement and evaluation or research activities as identified by the needs of the service area(s).

Master's degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW).

3 to 5 years of direct clinical experience in adult complex, chronic disease. Strong, demonstrated knowledge, skills and experience in assessment, formulation, diagnosis and treatment skills. Advanced critical thinking, advocacy, and collaborative problem-solving skills. Strong expertise understanding of navigating disability supports, including Assured Income for the Severely Handicapped, Canda Pension Plan Disability, Employment Insurance Benefits, Disability Tax Credit, and Alberta Works. Effective case management skills. Ability to work effectively with the team as well as independently. Ability to take initiative and demonstrate reliability and flexibility. Excellent communication, interpersonal and organizational skills. Adherence to AHS policies. Experience working with diverse populations. Proficient in using Windows, E-mail, Microsoft Office (including Excel) and typing. Experience with Connect Care.

Understanding in the use of ACEs. Experience and training in Collaborative Problem Solving.
