Description
Come join our amazing Central Zone Rural Team as a HCA Team Lead in Lacombe Continuing Care. As a member of the Seniors Health management team, the HCA Team Leader is responsible for planning, organizing, implementation, monitoring and evaluation of the HCA team. This position includes recruitment, supervision, education, mentoring and performance management of Health Care Aides in the Long-Term Care environment.
As the HCA team Lead your key responsibilities will be to provide guidance, supportive mentoring and on-site training for delegated procedures. Evaluating care given according to Health Care Aide competencies. Identifies need for replacement or creation of a position. Completes recommended posting and approvals in RMS system. Screens and shortlists applicants and interviews candidates with care manager as needed. Conducts reference checks and hires appropriate candidate. Reviews charting and related resident care and suggests areas of improvement or focus. Conducts annual and probationary performance conversations. Consults with the LTC Care Manager and Human Resources regarding potential disciplinary concerns. Works with HR on progressive discipline for identified staff, conducting interviews, and issuing disciplinary documents. Monitors time and attendance and resolving related issues. Registers staff in Attendance Management programs and conducts formal and informal meetings on attendance. Reviews and investigates MSN and RLS reports submitted by staff. Communicates trends and actions to the whole management team as appropriate. Contributes to enhancing Health Care Aide programs within the Zone. Promotes and supports AHS Values within the team and unit. Attends and participates in Out-of-Scope Manager team meetings and activities.
Licensed Practical Nurse Active registration in good standing with the CLPNA Minimum 5 years of LPN experience. Preferrable within Continuing Care environments. Familiar with computer applications, Connect Care facility living Nurse, RMS, ESP, outlook, TEAMS
