Description
The successful applicant will work in compliance with all relevant and applicable legislation and Hospital Policies and Procedures, will practice the Bluewater Health Standards of Performance Excellence and will fulfill their role within the Attendance Support Program parameters. The successful applicant will also possess effective communication and interpersonal skills, will have the ability to work independently and as part of a team, and will have strong attention to detail and accuracy.

• Graduation from a Faculty of Pharmacy of a recognized university required
• Licensed by the Ontario College of Pharmacists - Part A required
• Doctor of Pharmacy (Pharm D) degree preferred
• Completion of an Accredited Canadian Pharmacy Residency (ACPR) certificate preferred
• Proficiency using the MS Office Suite of products required
• Previous experience with Pharmacy software (i.e. Meditech, OPIS, Kroll, Pyxis) preferred
• Previous experience in a Hospital Pharmacy preferred
• Experience in Oncology pharmacy preferred
• Experience in Mental Health pharmacy preferred
• Previous experience in a Retail pharmacy setting asset
• All staff pharmacist duties which may also include the oncology or mental health roles in the outpatient pharmacy
• Reviews all medication orders for technical accuracy to ensure the medication order is authentic, accurate and appropriate and enters into computer system-Assesses the physician's medication order for therapeutic appropriateness utilizing the patient's medication profile and\or chart for the detection of duplicate therapy, potential allergic or adverse reactions, possible drug-disease incompatibilities, significant drug interactions or incompatibilities and suggest alternate drug therapy or pharmaceutical care plan when appropriate
• Monitors patient response to drug therapy for effectiveness and\or adverse effects and recommends changes in therapy where necessary. Monitors lab results and other clinical indicators for effectiveness of therapy and appropriateness of dose and frequency of medications
• Applies antimicrobial stewardship to all antimicrobial orders including therapeutic drug level monitoring
• Consults with and advises medical staff personnel regarding utilization of drugs and pharmaceutics. Provides information to physicians, nurses and other staff members on availability of drugs, indications and dosages, warnings on currently marketed drugs, incompatibilities of certain drugs, and contraindications of drugs and other pharmaceuticals.
• Counsels patients and family on use of various medications including dosage, side effects and correct administration techniques
• Provides direction and guidance to Pharmacy Technicians
• Enhances professional growth and development through participation in educational programs current literature review, in-service meetings and workshops
• Participates in multidisciplinary rounds, hospital committees and other meetings as required
• Participates in unit/department and Hospital Quality Improvement initiatives, projects and order set review
• Completes continuing education requirements as necessary
• Other related duties as assigned
