The Centre for Addiction and Mental Health has partnered with the Regional Geriatric Program of Toronto (RGP)Psychogeriatric Resource Consultation Program (PRCP) in the Greater Toronto Area (GTA) to provide knowledge-to-practice (KTP) services. These services are provided to individuals in long-term care homes and community service agencies that are caring for seniors with behavioural disorders associated with a dementia and other mental and neurological illnesses, and vulnerable adults who may be less than 65 years of age but whose cognitive and functional abilities are compromised by early onset dementia, developmental disability or acquired brain injury. The CH&E Coordinator/Psychogeriatric Resource Consultant (PRC) position liaises with appropriate ‘in-house’ services and local psychogeriatric outreach and Behavioural Supports Ontario (BSO) services. The Coordinator is part of a team of PRCs distributed across the region.

While based at CAMH, the consultant’s work will be focused in long-term care homes and community service agencies in an assigned catchment area.

Core functions of the role includes:

Reporting jointly to the CAMH Manager of the Geriatric Mental Health Outpatient Clinics and the Director of the RGP, the candidate will support a healthy workplace that embraces diversity, encourages teamwork and complies with all applicable and regulatory requirements.

The CH&E Coordinator/PRC will be required to provide services in accordance with the PRCP’s KTP framework leading to practice and workplace improvements within the serviced organizations and enhance service integration. The CH&E Coordinator/PRC will also optimize opportunities to collaborate with other PRCs, and participate in the program’s accountability initiatives. The Consultant is responsible to adjust her/his work schedule to meet the needs of their assigned organizations.
