Description
As a casual Clinical Pharmacist, you will provide a full range of clinical and distributive services to inpatients and outpatients within the context of a computerized environment; and with clinical expertise in psychiatry and addictions to provide safe and effective medication management to the patients of CAMH. You will work collaboratively with other pharmacy team members and health care providers. Duties include evaluating medication orders, developing pharmaceutical care plans, resolving drug therapy problems, collecting best possible medication histories, facilitating medication reconciliation, and ensuring accurate dispensing of pharmaceuticals in an interdisciplinary team setting. The position also supports and participates in risk management, patient safety, clinical knowledge sharing and quality improvement activities. You will also teach, mentor and transfer knowledge to students, residents, and interns, while supporting the department’s commitment to education and research. You possess a B.Sc. in Pharmacy and are registered with the Ontario College of Pharmacists (Part A) and two years of relevant pharmacy experience. Experience working within a mental health setting or possession of a Hospital Pharmacy Residency and/or a postgraduate pharmacy degree would be a definite asset. You will have knowledge of computerized unit dose drug distribution system and in-depth knowledge of pharmacy operations and medication-use processes. Understanding of the clinical and distributive activities associated with medication use is required as is knowledge of medication safety risk in health care. Analytical skills are required including the ability to assess and take responsibility for patient drug-related needs to offer value-added solutions. The successful candidate must also possess excellent communication (both oral and written) and interpersonal skills to work effectively with the patient population. The successful candidate must have demonstrated organizational skills as well as problem recognition and problem solving skills. Candidates will possess the ability for self-directed learning and working. Candidates require the ability to work effectively with individuals from diverse backgrounds. Bilingualism (French/English) and/or proficiency in a second language would be an asset.
