Registered Nurse - Mobile Nursing Team-camh

CAMH is currently seeking two (2) part-time, permanent Registered Nurses to work in the Mobile Nurse role. In this role, you will be responsible to support point-of-care teams with medically compromised clients; support treatment and complex care planning, and provide just-in-time staff education. You will work within a changing environment with inter-professional teams to advocate for and promote wellness while ensuring the delivery of safe, effective, and ethical nursing care, consistent with the College of Nursing (CNO) standards. You will also support the hospital’s Code Blue response team.

Reporting to the Director of Practice Innovation, Standards and Measurement in the Professional Practice Office, you will support CAMH’s Clinical Acute Care, Early Child/ Emerging Adults, Addictions and Complex Care and Recovery Programs vision of service excellence for clients with complex medical needs. Your primary role will be to provide holistic, evidence-based, culturally appropriate, patient-centered quality care in accordance with the College of Nurses of Ontario standards and CAMH vision, values, and policies.

In this consultation role, you will provide education and/or clinical care support where required for the patients; liaise with the unit staff regarding patient's condition, change of status; and request/complete appropriate interventions related to patient care issues such as complex wound care, suture removal, palliative care, pain and symptom management, urinary catheter management, stoma management, diabetes management, phlebotomy, IV therapy, 12 Lead Echocardiogram, point of care testing and Code Blue Response, and other duties as assigned. Further, where appropriate, you will support the inter- and intra-hospital admission, discharge, and/or transfer process of patients to ensure a warm handover, and provide follow-up support to hospitalists and staff for patients being transferred inter- and intra-hospital units. You will also share your expertise through education for all members of the inter-professional team and offer educational placement opportunities to students to facilitate their learning and development in collaboration with the Integrated Care Nurse Educators.

You will support and contribute to a healthy workplace environment that embraces diversity, encourages teamwork and complies with all applicable regulatory and legislative requirements. You will be located at Queen Street site, but may be required to travel to other sites as needed. This job requires coverage for 09:00-19:00 Monday to Friday. However, as the program develops and progresses, these hours may change and may require coverage for evenings, weekends and stat holiday shifts.

The successful candidate must be a Registered Nurse and member in good standing with the College of Nurses of Ontario and possess a current B.C.L.S. (CPR). Preference will be given to candidates who possess, or are currently pursuing a B.Sc.N. Primary Care, Community, Medical/ Surgical or Medical Emergency nursing experience is required. Mental health and addictions experience is preferred. Computer literacy is also required. Relevant knowledge, skills and competence including IV therapy, catheterization, ECG assessment and wound care is also preferred. Strong analytical, organizational, conceptual and decision-making skills are required, as well as demonstrated flexibility and the ability to adapt and manage changing priorities. You will possess effective decision- making, problem solving, proven organizational skills, and the ability to facilitate work within a collaborative, inter-professional environment. Demonstrated superior written and verbal communication and interpersonal skills is required. You will have proven planning, time management, organization and coordination skills. You will have excellent care coordination skills with services in the hospital and community as well as skills in triaging patients in person and by telephone. You will have demonstrated patient/family teaching experience as well as experience working with diverse clients and stakeholders.

Please note: These two (2) part-time, permanent positions are a 0.5 commitment, which may be subject to change as per the Collective Agreement. Additional hours may be available based on operational needs. This position is part of the ONA Bargaining Unit.
