Description
The Complex Care & Recovery Program (CCR) is seeking a full-time, temporary (1 year), Registered Nurse who will support the Psychosis Coordinated Care Service (PCCS or formerly known as PHP) and the General Psychoses Service Injection Clinic. The role will offer an opportunity to support clients transitioning from inpatient units and/or preventing inpatient admission of the Psychosis Recovery Treatment outpatient clients who require stabilization of acute psychotic symptoms. PCCS provides case management support, medication administration and offers recovery-focused, therapeutic programming based on HQO standards.
You will work in collaborative partnership with clients, their formal and informal supports and the family health care teams using strength-based principles to support the successful transition of care. You will be expected to work as a member of the interprofessional team, perform ongoing psychiatric and physical assessments, provide medication administration, develop, implement and evaluate inter-professional service plans for clients who have schizophrenia, primary psychotic disorder and concurrent disorders. You will be able to work with clients from diverse populations; participate in clinical supervision or reflective practice; contribute to program development; provide education support and supervision to students and participate in the research activities of the program. Your skills include the ability to make behavioural observations, complete mental status and physical exam assessments, the ability to monitor clients’ response to pharmaceutical and care interventions.
You will have sound knowledge of community resources, crisis intervention, recovery, trauma-informed care, addiction issues and severe and persistent mental illness. You will support and contribute to a workplace that embraces diversity, encourages teamwork and complies with all applicable legislative and regulatory requirements. This position is located at 1051 Queen Street West.
The successful candidate must be a member in good standing with the College of Nurses of Ontario and possess a current B.C.L.S. (CPR). You will have two years of nursing experience. Preferred experience in psychiatric/mental health nursing. Experience within an outpatient setting with sound knowledge in crisis intervention and in rapid intake assessment is an asset. Preference will be given to candidates who possess or are currently pursuing a B.Sc. N and/or CNA certification in Psychiatric/Mental Health Nursing. Preference will also be given to those with community nursing experience, and a comprehensive understanding of Recovery principles and their application to client service. You will possess excellent communication and interpersonal skills to enable you to develop the Nurse-Client relationship and work within a collaborative and diverse inter-professional team. Effective leadership, decision-making, organizational, and problem solving skills are required. Knowledge of emergency psychiatry and crisis intervention principles and theory are required. The ability to work with clients of diverse cultural and social backgrounds is necessary. Computer literacy is required. Bilingualism (French/English) or proficiency in a second language would be an asset.
