Team Lead - Bridging Service-camh

Reporting to the Manager of the Bridging Service, and working collaboratively with the Acute Care practice leadership group, the Team Lead supports the Manager to anticipate needs of the outpatient clinic over time and collaborates with the clinic’s interprofessional team to provide effective coordination of client/patient-centered care, consistent with CAMH’s vision and strategic directions.

The Team Lead will assume responsibility for coordinating patient flow, supporting treatment planning, leading special initiatives and group discussions, and supporting complex clinical scenarios.

The successful candidate will coordinate with the Manager regarding staff schedules to ensure appropriate coverage of the clinic and equitable distribution of caseload. In collaboration with practice leads you will link new staff, with appropriate clinic resources and facilitate orientation to the clinic, ensuring they become familiar with the clinic’s procedures/processes. You will coordinate and/or assign, in collaboration with clinical supervisors/preceptors, an appropriate CAMH staff to assist students of all disciplines in providing a safe and respectful learning environment. The Team Lead will collaborate with the Manager regarding staff participation in meetings/ trainings, educational events.

As part of the Team Lead’s responsibility, the successful candidate will provide direct clinical care and liaison and coordination of client/patient discharges.

The position is located at 1051 Queen Street West and will be a regular day shift.

The successful candidate must be registered in good standing with the College of Nurses of Ontario. A current B.C.L.S. (CPR) certificate is also required. Preference will be given to candidates who possess or are currently pursuing a BScN and CNA certification in Psychiatric/Mental Health Nursing. At least two years (or 3900 hours) of clinical experience within the mental health and additions field is preferred. The successful candidate will possess proficiency and leadership in problem-solving, decision-making, conflict resolution, negotiation, communication, delegation, critical thinking, team building and interpersonal skills. The successful candidate will possess the ability to think at a program-level and participate in unit-level as well as organization-wide change initiatives. The individual will have the ability to communicate effectively with the team, including management, both verbally and in writing; knowledge of the roles of interdisciplinary team members, their professional responsibilities and areas of expertise is essential. Demonstrated expertise in maintaining a culture of safety for staff, clients/patient, families and visitors according to CAMH and regulatory standards is critical. Bilingualism (French/English) or proficiency in a second language would be an asset.
