Reporting to the Manager of Psychosis Coordinated Care Service (PCCS) and working collaboratively with the Psychosis Recovery & Treatment Program, Practice leadership group, the full-time, temporary (1 year) Team Lead supports the Manager to anticipate needs of the outpatient clinic over time and collaborates with the clinic’s interprofessional team to provide effective coordination of client/patient-centered care, consistent with CAMH’s vision and strategic directions. The Team Lead will provide triaging of referrals including rapid assessments of clients, schedule intake assessments for PCCS and support triaging of General Psychosis Service (GPS) referrals in collaboration with the physicians. The Team Lead will assume responsibility for the communication, liaison and coordination of client/patient admissions, discharges, transfers, and support client assignments. The successful candidate will facilitate service delivery, ensuring a high standard of quality and safety. The Team Lead will ensure that the client/patient assignment is clear and reflects the principles of accountability for care, also ensuring that client/patient assignments are accurate and properly recorded as per established clinic’s processes and CAMH standards.
The Team Lead will work with the Manager to coordinate staff schedules to ensure appropriate coverage of the clinic and lead daily/weekly team and treatment planning meetings. You will link new staff with appropriate clinic resources and facilitate/support alongside staff mentor and Advanced Practice Lead orientation to the clinic, ensuring they become familiar with the clinic’s procedures/processes. You will coordinate and/or assign, in collaboration with clinical supervisors/preceptors, an appropriate CAMH staff to assist students of all disciplines, educating them on team procedures, and to assist with client/patient care issues. The Team Lead will collaborate with the manager regarding staff participation in off-unit meetings/ training, educational events such as Nursing Practice Council, Education Rounds, and Discipline Councils. You will ensure that the monthly admission, transfer and discharge reports (Patient Flow) are accurate and follow appropriate clinic procedures, transfer of care report and other relevant reports, as required.

The successful candidate must be a member in good standing with the College of Nurses of Ontario. A current B.C.L.S. (CPR) certificate is also required. Preference will be given to candidates who possess or are currently pursuing a BScN and CNA certification in Psychiatric/Mental Health Nursing. At least two years (or 3900 hours) of clinical experience within the mental health and additions field is preferred; with a particular focus on clients with schizophrenia and specific knowledge of the recovery-based model of care. The successful candidate will possess and practice strength-based approach, skills, proficiency and leadership in problem-solving, decision-making, conflict resolution, negotiation, communication, delegation, critical thinking, team building and interpersonal skills. The successful candidate will possess the ability to think at a program-level and participate in clinic-level as well as organization-wide change initiatives. The individual will have the ability to communicate effectively with the team, including management, referral sources both verbally and in writing; knowledge of the roles of interprofessional team members, their professional responsibilities and areas of expertise is essential. Demonstrated expertise in maintaining a culture of safety for staff, clients/patient, families and visitors according to CAMH and regulatory standards is critical. Bilingualism (French/English) or proficiency in a second language would be an asset.
