SUMMARY OF THE POSITION
A person who assumes responsibility for a nursing care program based on the bio-psycho-social needs of users or groups of people entrusted to his/her care. He/she assesses the state of health, determines and ensures the implementation of the therapeutic nursing plan, the nursing care and treatment plan for users with complex health problems or varied bio-psycho-social dimensions. He/she initiates and provides nursing and medical care and treatment to maintain or restore health and prevent illness. He/she may prescribe laboratory analyses, treatments, products, drugs and dressings in the fields of wound care, public health and routine care, in accordance with current regulations.

He/she designs, implements and evaluates care programs requiring advanced knowledge, targeting complex health problems or involving varied bio-psycho-social dimensions. He/she participates in the development or adaptation of clinical and care quality assessment tools. He/she coordinates teamwork and plays an advisory role to the care and interdisciplinary teams.

Identifies user needs and interventions requiring coordination of services within the facility and between different facilities or organizations in the community. Where necessary, he/she coordinates these services.

He/she provides orientation for new staff and participates in the training of trainees. He/she supervises teaching for users, their families and groups of people in specific programs, and collaborates on research.

SPECIFIC FUNCTIONS* Act as a liaison between the client, the referring physician, the team at the Miyupimaatisiiun Community Centre (CMC), the different departments involved at the CRDS level (Imaging, Laboratory, General Practitioners, Specialists, MSSS, MUHC, other CRDS), the answering physician, and other health professionals (social worker, nutritionist, physiotherapist, and other community health partners) in order to adequately address the patient's needs;* Participate in the clinical validation of consultations and evaluate service requests when the reason for consultation is not standardized, and redirect consultations to other services if necessary;* Monitor the Safir platform - Digital Advice and intervene as needed;* Participate in the deployment or implementation of new projects in Region 18 related to the CRDS, issued by the MSSS;* Participate in the clinical validation of consultations for specialized services;* Extract information from the DSQ (Dossier Santé Québec) for physicians and healthcare institutions to assess a medical record;* Ensure and maintain appropriate and effective communication between the patient, the specialist, and the Miyupimaatisiiun Community Centre (CMC) team;* Monitor prescribed waiting times for consultations and ensure follow-up with the relevant entities if necessary;* Participate in the development and review of clinical tools (algorithms, simplification of CRDS forms, workflows, procedures, protocols) related to the CRDS and associated platforms according to clinical conditions, and provide documentation to support the CRDS team and users;* Document interventions and observations in the form of electronic notes;* Promote collaboration among various stakeholders to find solutions;* Ensure that patients receive quality care and take the necessary measures to guarantee their satisfaction;* Disseminate appropriate and accurate information to support informed decisions;* Implement corrective actions and follow up as needed following an incident/accident;* Contribute to the maintenance, organization, and accessibility of services offered to clients;* Sort APSS consultations or other consultations with the support of specialists from Eeyou Istchee;* Provide appropriate and accurate information to support informed decisions;* Participate in relevant meetings related to the CRDS and specialized services;* Perform the usual duties of a nurse when required.

Education:

  • Member in good standing of the O.I.I.Q;
  • Bachelor of Science in Nursing or an undergraduate degree consisting of three (3) admissible certificates, of which two (2) are accredited certificates in Nursing.

Experience:

  • Minimum of two years of experience in different sectors of a hospital.

Knowledge and Abilities: * Knowledge of Cree culture and social and health issues in Eeyou Istchee (an asset); * Knowledge of the CRDS and the service corridors in Eeyou Istchee;* Good knowledge of IT tools (OmniLab, Care4) (an asset);* Excellent interpersonal and teamwork skills; * Good resistance to stress; * Excellent communication and listening skills;* Excellent critical thinking, synthesis, organizational and decision-making skills; * Take initiative and be autonomous; * Open to other culture, ability to adapt to changes and new environment;* Show interest and empathy towards others; * Possess a capacity for analysis and synthesis.

Language:

  • Fluent in English and French;
  • Fluency in Cree is an asset.

Other:

  • Available to travel inside and outside of the Eeyou Istchee territory as required;
  • Possibility of telework;
  • This position offers the possibility of telework, depending on operational needs and after agreement with the employer. Please note that this is not a permanent condition of the position and the employer reserves the right to request regular physical presence in the office.
