Job Description

Qualifications/Education

· Diploma, Registered Practical Nurst with medication certificate or with full scope of practice and currently registered as an RPN in good standing with the College of Nurses of Ontario

· Computer literate in Word, Excel spreadsheets

· Current CPR and FirstAid Certificates

· Training in P.I.E.C.E.S. ™, Gentle Persuasive Approach, U-FIrst!, Montessori

· A.I.S Education Modules completed

Experience

· Prior experience with clients in long term care and the community

· One (1) year recent experience as an RPN in long-term care is preferred

· Experience with quality improvement process an asset

· Experience with documentation, Care Plans, BATT Tools, DOS, Metrix

Knowledge

Demonstrates knowledge of dementia, delirium, mental health issues in delivery of care and its effect on the person and family members or partner in care, include knowledge of:

a. Most prevalent types and related causes

b. Disease processes, stages and progression

c. Diagnostic and assessment process

d. Cognitive or neurological symptoms

e. Current treatment interventions

f. Communication skills appropriate to needs of the person

g. Strategies to promote optimal quality of life

h. Experience of disease from the perspective of the person, family members or other partners in care

Person-Centered Care

Delivers person-centered care which recognizes both the uniqueness of each person and an awareness of one’s own contribution to that relationship, including personal attitudes, values and actions, to include:

a. Contributing to the development of the person-centered philosophy of care

b. Promoting and preserving the abilities and self-esteem of the person

c. Promoting the person’s integration to their environment

d. Using effective communication and interpersonal skills when interacting with the person, family members or partners in care and other care providers.

Clinical skills

Conducts an assessment and describes interventions with respect to the behaviors of persons, including:

a. Recognizes that most observable behaviors have meaning: therefore, the etiology of the behaviors must be assessed and accounted for in the caregiving process

b. Assesses the meaning, etiology and inherent risk of behaviors using an objective, systematic and holistic process that takes into account the physical, intellectual, emotional and functional capabilities of the person, as well as the environmental and social aspects of their surroundings

c. Identifies caregivers' strategies that are abilities focused, person-centered and age appropriate for responding to behavior and managing associated tasks

d. Focuses on prevention of responsive behaviors by relating well, manipulating the social and physical environment, focuses on persons abilities and knowing the individual and their life story and aspirations

General Requirements:

· Current registration and certificate of competence as a Registered Practical Nurse with the College of Nurses of Ontario

· Current certificate of an approved drug administration or equivalent course

· Nursing experience in long term care required

· Knowledge and experience in written documentation requirements for interdisciplinary progress notes and care plans required

· Mentally and physically able to perform duties assigned

· Ability to communicate positively with other staff, residents and families

· Demonstrate empathy, concern and interest in providing quality care and service to residents

· Demonstrated organizational skills and ability to complete tasks in a timely manner

· Ability to work independently and as part of a team

· Ability to perform all duties in a safe and efficient manner in accordance with the applicable Legislation, Job routines, the College of Nurses Standards of Nursing practice, Extendicare’s Resident care manual and quality care Standards

· Flexible to work in all areas of the home

· Participate in Quality meetings

· Good Attendance is a must
