REGISTERED PRACTICAL NURSE with Behavioural Support Position
Extendicare Lakefield - Full-time
Registered Practical Nurses are invited to apply for the permanent full-time (75 hours bi-weekly) position with Behavioural Support focus to work at our Extendicare Scarborough, a 100-bed long-term care home, located in Lakefield, Ontario.
Reporting to the Assistant Director of Care, the RPN will enhance the effective management and quality of nursing practice in keeping with the Standards of the College of Nurses of Ontario, Long Term Care Standards and Extendicare Standards.
RESPONSIBILITIES
Works in a team environment within the guidelines of the Central East LHIN Behavioural Support Ontario (BSO) Project to assist with coordinating the care and services for residents with responsive behaviours.
Demonstrates knowledge in dementia care, delirium, mental health issues and the impact that is has on the resident’s quality of life, family members and care partners.
Responsible for promoting and contributing to the development of a person-centred philosophy of care for residents exhibiting behavioural challenges and utilizes effective communication and interpersonal skills to collaborate with the resident, family, staff and care providers to ensure that a holistic plan of care is in place for the resident.
QUALIFICATIONS
- Appropriate current certificate of competence from the College of Nurses of Ontario required.
- Registration or eligible for registration with the Registered Practical Nurses Association of Ontario.
- Previous leadership experience in long-term care preferred.
- Experience dealing with the cognitively impaired.
- Comprehensive knowledge of nursing and health care practices, as well as knowledge in Infection Control Practices.
- Demonstrated commitment to working with seniors.
- Ability to work effectively individually and with others while prioritizing tasks to handle the most urgent first.
- Ability to present information clearly and effectively both verbally whether in person or over the phone, and in writing; an active listener who projects a friendly demeanor.
- Ability to collect and use information to thoroughly think through problems and situations; puts information from various sources together to see the ‘big picture’, and generate creative solutions.
- Organizational, planning, time management and multi-tasking skills. Must be able to work and provide guidance to staff in all shifts.
- Understands and upholds all rules and requirements under applicable legislation (including but not limited to the Employment Standards Act, Human Rights Code, Ontario Labour Relations Act, the Ontario Health and Safety Act, Long Term Care Homes Act).
Utilizes effective communication and interpersonal skills when interacting with the residents, family members and internal and external care partners.
Delivers person-centred care which recognizes both the uniqueness of each resident and an awareness of one’s own contribution to the relationship, including personal attributes, values, and action to include:
- contributing to the development of the person-centred philosophy of care.
- promoting and preserving the abilities and self-esteem of the person.
- promoting the residents’ integration to their environment.
- developing strategies to promote optimal quality of life for the resident with responsive behaviours.
Uses an assessment tools based on best practices to conduct initial and ongoing assessments describing interventions with respect to the behaviours of the residents including:
- Documenting observational behaviours, recognizing that they have meaning, assessing and accounting for how the behaviours impact the care delivery process.
- Assesses the meaning and identifies the behavioural risk using objective, systematic and holistic process that take into account the P.I.E.C.E.S.
- Identifies caregiver strategies that are resident-centred, age appropriate, maintains the resident’s dignity when responding to the behaviours and managing the associated risks.
- Conducts weekly rounds with the PSW Behavioural Support staff and P.I.E.C.E.S. Nurse.
- Ensures that there is a current plan of care in place for the resident with responsive behaviours that directs the staff in providing care and mitigating risk to the resident, other residents, staff, and care providers.
- Liaisons with external partners to support resident-centred care.
- Communicates to senior management staff any areas of risk associated with responsive behaviours.
- Participates in quality improvement activities and takes part in reporting monthly stats on responsive behaviours.
- Acts as a mentor and coach to staff to ensure collaboration of care and services for responsive behaviours.
- Conducts resident case reviews to support the plan of care for the responsive resident.
- Attends Professional Advisory Committee on a quarterly basis.
- Knowledge of how medication treatment may affect the care of the residents and collaborates with the pharmacist and physician.
- Assumes responsibility for personal and development through participation in Staff Development programs, seminars and workshops. Required to attend ongoing training to support skills in managing residents with responsive behaviours.
- Demonstrates an understanding and commitment to the philosophy, objectives, and policies of the Home and the Behavioral Support Ontario Project.
- Maintains good relationships with residents, staff, and the general public.
- Provides quality customer service to all parties, including but not limited to: the Management Team, Head Office, other staff, the families and the residents.
- Completes all tasks, duties and documentation with efficiency and professionalism.
- Works flexible hours to meet behavioural needs of the resident exhibiting responsive behaviours.
- Required to attend behavioural support committee meetings as required.
- Receives direction, assignment and evaluation of work from the Assistant Director of Care and Social Worker and provide feedback.
- Advises PSW on their work and duties.
- Abides by all Extendicare policy and procedures and other duties and tasks as assigned.
