Job Description

The HCA provides direct personal support and care to patients to fulfil the physical, intellectual, emotional, social and spiritual needs of patients in accordance to polices, procedure and work routines. Assists in providing direct care such as ambulation, feeding, bathing and grooming.

Qualifications
1) Health Care Aide certificate from a recognized health care aide program and are registered on the HCA Directory.
2) Demonstrate able to read and write as well as to understand verbal and written instructions in English.
3) Demonstrated reliability in assuming/ carrying out the commitments and obligations of the position with minimum supervision.
4) Knowledge / experience working with residents who have a diagnosis of complex medical needs.
5) Recent experience working with in the general programs an asset.
6) Ability to promote resident independence
7) Capable to work busy assignment with multiple mechanical lifts.
8) Participate in Care Planning process by identifying and implementing ideas to meet resident’s physical, social, emotional, intellectual, and spiritual needs
9) Become a care partner to each resident and demonstrate the ability maintain positive working relationships with families and staff.
10) Demonstrate flexibility and a satisfactory employment record including completion of mandatory annual education
11) Strong customer focuses with an excellent attendance record is required

Responsibilities:

1) Directly accountable to the Program Manager and under the direction of the nurse.

2) Become a care partner to assist with resident care helping residents to maintain independence and providing ADL supportive care as directed.

3) Performs all duties in accordance with established health and safety regulations/guidelines, policies and procedures.

4) Participates in program activities, best practice initiatives, and in the development of action plans.

5) Responsible for attending mandatory education and recognize and pursue self-development opportunities.
