Extendicare New Orchard Lodge is seeking self-directed, results oriented nursing leader, with strong communication and critical thinking skills. You will provide innovative and creative clinical leadership for our inter-professional team to build capacity and support knowledge development in geriatric care, and work collaboratively with our physicians, staff, and system partners.
The Nurse Practitioner is an integrated member of the leadership team and values the importance of working within a strong interdisciplinary team. Reporting to the leadership team, you will collaborate with the Administrator, Director of Care and Medical Director and will provide direct comprehensive care, to Long-Term Care (LTC) residents with specialized and complex needs based on enhanced clinical knowledge and skills. The Nurse Practitioner practice is an interdependent practice in which the Nurse Practitioner works collaboratively within an interdisciplinary team, however, the scope of the NP’s practice will be beyond that of regular nursing care (full scope of extended class nurse as per the College of Nurses of Ontario (CNO)).
RESPONSIBILITIES
-Delivers primary health care to residents in long-term care in the management of acute and chronic medical conditions, therapeutic management, health promotion and disease/injury prevention to deliver comprehensive health services
-Provides clinical leadership to LTC team members centered around resident care needs which can include resident admission/discharge, hospital discharge planning, palliative and end-of-life care, pain management, dementia care and responsive behavior management
-Conducts health assessment using and adapting assessment tools and techniques based on resident needs and performs a completes health history, including physical, psychosocial, emotional, cultural and ethnic dimensions of health
-Performs physical examination and identifies and interprets normal and abnormal findings and orders diagnostic investigations and interprets results using evidence-based clinical reasoning
-Collaborates and engages with residents and family members to develop person-centered plans of care and synthesizes health information using critical inquiry and clinical reasoning to formulate a diagnosis, health risks and states of health/illness
-Communicates with residents about the health assessment findings and/or diagnosis, including outcomes and prognosis through the application of knowledge of pathophysiology, psychopathology, epidemiology, infectious diseases, behavioural sciences, and family processes when making diagnoses and providing overall therapeutic management
-Selects appropriate interventions from a range of non-pharmacological and pharmacological interventions to restore or maintain resident functional, physiological, and mental stability to achieve optimal health
-Promotes safe resident care by mitigating harm and addressing immediate risks for residents and others affected by adverse events and near misses
-Liaises with discharge planning services for hospitalized residents to ensure a smooth and safe transition back to the long-term care home (LTCH)
-Collaborates with Extendicare Leaders and community health services in initiatives for the development, implementation, and evaluation of systems to promote continuity of resident care across the continuum and collaborates with the leadership team on educational programs for the care team such as participating in professional advisory, and leads and supports quality improvement and research initiatives
-Dedicated time of 80% of the NP’s time to be engaged in direct care to residents, and 20% to be engaged in research and providing education to improve resident care
-Ability to incorporate knowledge of diversity, cultural safety, and the determinants of health in the assessment, diagnosis and therapeutic management of residents and the evaluation of outcomes and exercise independent judgment and to assess situations and problems efficiently and effectively
-Leadership skills, working well within an inter-professional team and ability to foster effective working relationships and ability to provide clinical supervision, education and mentoring for nursing students, medical students, and other learners, including residents/families
-Ability to build and maintain effective relationships and partnerships with a diverse range of stakeholders as well as work effectively within multidisciplinary teams
-Strong and effective verbal and written communication skills
-Ability to work with flexibility for potentially evenings or weekends participates in providing after hours and on-call coverage
-Knowledge of RNAO Best Practice guidelines and MLTC Regulations and Act
QUALIFICATIONS
-College of Nurses of Ontario registration in the Extended Class
-Baccalaureate in Nursing (BScN), Masters Degree in Nursing (MScN or MN) an asset
-Membership with Nurse Practitioners’ Association of Ontario (NPAO) (preferred)
-Has acquired professional liability insurance (with either RNAO or other liability insurance)
-National certification in gerontology an asset
