Job Description

REPORTING TO: Nursing Coordinator

SUMMARY OF DUTIES:

Under the direction of the Nursing Coordinator, or designate, is responsible for the care of the residents in keeping with the philosophy and policies of Extendicare and carrying out nursing, resident care and administrative duties in the Nursing Department as directed. Abides by facility and Nursing Department policies and procedures, communicates and interprets them to other staff, residents, medical staff, visitors and the public. Organizes and prioritizes responsibilities effectively on a daily basis.

Consults with and advises the Director of Care concerning resident care matters.

Supervises Licensed Practical Nurses and Health Care Aides.

May be responsible for the general operation of the facility in the absence of the designated responsibility person, with the support and guidance of the Admin-on-call.

Effectively uses the nursing process;

Develops individualized care plans for each resident, based on assessed individualized needs; Sets realistic short and long term goals Identifies appropriate nursing actions; Implements plan of care; Evaluates nursing care and effectiveness of the plan of care by regular review.

QUALIFICATIONS:

Current registration status with the Manitoba Nurses Association of Registered Nurses or the Registered Psychiatric Nurses’ Association of Manitoba. Must be knowledgeable in principles and philosophy of long term care; Mentally and physically able to perform duties as outlined. Ability to supervise and direct the work and learning of others. Demonstrates concern and interest in providing quality care to residents. Ability to work independently with minimal supervision and complete assignments in a timely manner Ability to communicate effectively with other staff, residents, families and the public and community resource personnel

TYPICAL DUTIES AND RESPONSIBILITIES:

Performs basic nursing skills as listed in the Manitoba Association of Registered Nurses Standards of Practice.

Assists in the development of and maintenance of care standards and objectives.

Assists in the development of and ongoing review of resident care plans which include rehabilitative and restorative objectives. Maintains proficiency in use of the MDS/RAI program.

Maintains resident health care health care records and performs charting duties ensuring that records are informative, accurate and current.

Admits new residents and orientates new staff and their families to aspects of the facility, as per the admission procedure.

Participates in the orientation of new staff and the ongoing instruction and supervision of all nursing staff. Promotes ongoing staff education.

Supervises and performs direct resident care in accordance with policies and procedures. Makes regular rounds, administers medications and performs professional treatment services as required or requested.

Schedules resident appointments, arranges for regular visits for the Podiatrist and other visiting professionals. Arranges transportation for residents to appointments. Prepares transfer forms as required.

Meets regularly with the consulting Pharmacist to review all resident medication profiles.

Responsible for contacting physician with resident concerns, transcribes physician orders, ensures physician’s telephone orders are co-signed, and regularly reviews treatments with attending physicians during scheduled rounds.

Makes arrangements for diagnostic procedures and examinations as ordered by physician..

Monitors the use and maintenance of supplies and equipment in the Nursing Department. Requisitions supplies and equipment when necessary.

Keeps informed of applicable legislation related to long term care, and facility policies and procedures. Communicates with staff through regular staff meetings.

Participates in formal and informal teaching programs for all nursing staff as well as health teaching to residents and families as appropriate.

Assesses all staff injuries and initiates appropriate action/treatment.

Regularly evaluates care staff and provides input into performance appraisals for Health Care Aides.

Promotes effective communication with relatives and other interested individuals as well as communication and cooperation with other departments.

Performs all duties in accordance with the written policies and procedures of Extendicare, the facility and the Nursing Department.

Participates in the evaluation of care and/or service provided by the Nursing Department by developing, implementing and evaluating activities under the Quality Assurance Program.

Performs all duties in a safe and efficient manner and maintains work area in a clean, orderly, and safe condition.

Follows manufactures instructions in the operation and use of all equipment and chemicals.

Alert to potential health and safety hazards and takes appropriate action. Reports unsafe and hazardous working conditions and any equipment malfunctions to Administration.

Knowledgeable of facility procedures for fire and environmental safety.

Performs duties in accordance with prescribed sanitation and infection control procedures.

Respects residents rights to dignity and privacy and performs duties in a courteous and respectful manner.

Adheres to “Standards of Employee Conduct,” “Commitment to Residents”, “Bill of Rights”, and “Extendicare Quality Care Standards.”

Works closely and cooperates with other staff and residents. Maintains good working relationships

Participates in inter-disciplinary care conferences, committees, meetings, as required.

Responsible for the development and upgrading of his/her knowledge and skills. Attends inservice sessions and conferences as required.
