Job Description

  • Conduct daily rounds to assess residents with existing wounds and those at high risk for skin breakdown.
  • Check for any new skin issues or changes in current wounds.
  • Perform wound assessments, including measuring wound size and depth and noting any signs of infection.
  • Clean and dress wounds according to the care plan and best practice guidelines.
  • Apply appropriate wound care products and dressings based on the type and stage of the wound.
  • Document wound care procedures, including dressing changes and observations in the resident's medical record.
  • Meet with the interdisciplinary team, including family members, physicians, dietitians, and physical therapists, to discuss wound care plans and any changes in treatment.
  • Communicate with the Director of Care and nursing staff about residents' wound care needs and any required adjustments in the care plan.
  • Provide ongoing education and training to nursing staff on proper skin care techniques, early identification of skin issues, and wound care procedures.
  • Conduct in-service training sessions on pressure ulcer prevention, appropriate use
  • Educate residents and their families about wound care management, including proper hygiene, nutrition, and mobility.
  • Address any concerns or questions from residents and their families regarding wound care.
  • Reassess residents with complex wounds or those who require more frequent monitoring.
  • Evaluate the effectiveness of current wound care treatments and make necessary adjustments.
  • Consult with the healthcare team for signs of wound deterioration or infection.
  • Complete detailed and accurate documentation of wound assessments, treatments, and progress notes.
  • Update care plans based on the latest assessments and any changes in the resident's condition.
  • Ensure an adequate supply of wound care products and dressings.
  • Coordinate with the DOC clerk to order necessary wound care materials.
  • Monitor inventory levels and manage cost-effective use of supplies.
  • Review the day's activities and plan for the next day’s priorities.
  • Prepare for any scheduled wound care rounds or interdisciplinary meetings.
  • Ensure all documentation is complete and up to date.
  • Communicate between shifts with nursing staff, highlighting any critical wound care issues or residents needing close monitoring.
  • Communicate with the Director of Care about significant wound care concerns or developments.
  • Additional Responsibilities:

  • Conduct regular audits of wound care practices to ensure compliance with best practice guidelines and regulatory standards.
  • Identify areas for improvement and implement corrective actions as necessary.
  • Participate in quality improvement initiatives related to skin and wound care.
  • Collaborate with the quality assurance team to develop and implement strategies for reducing the incidence of pressure ulcers and other skin conditions.
