Job Summary

We are currently looking to fill a Casual opportunity for a Client Services Assistant to join our Home Health Team located in New Westminster, B.C.

Detailed Overview

Supports clinical staff in completing various contractual, financial and administrative aspects of client services; provides assistance to frail seniors and/or other adults with chronic health conditions requiring support and coordinated care to live successfully in the community by participating in care planning such as documenting client needs and coordinating services for clients and/or their caregivers; liaises with non-Fraser Health community services to facilitate client referrals to those services, where appropriate.


Responsibilities


  1. Coordinates financial, administrative and contract aspects of client services and care management under the direction of the Manager, Clinical Services or assigned clinical staff.
  2. Participates with health care provider in care planning with client by obtaining required client information from a variety of sources and documenting client needs/wants.
  3. Gathers and documents information from existing health information systems for all new clients for review by assigned health care professional; identifies additional avenues for gathering information such as First Nations, Department of Veterans Affairs and collects relevant and/or missing data as required; reviews files to obtain historical information.
  4. Works with the client and/or caregiver to complete the financial portion of the client's financial assessments and temporary rate reduction requests; works with the client/caregiver to document other personal information to complete waiver forms such as sponsorship or residency forms; coordinates contract, financial and administrative aspects of client services and care management under the direction of the Manager, Clinical Services or assigned clinical staff.
  5. Supports long term clients to access community or community related service services in accordance with the established care plan; ensures that the assigned health care professional is kept informed and reviews materials such as policy and procedures and clinical decision-making tools to assist with various care management issues.
  6. Researches community resources and identifies information that will assist in the development and implementation of client services and care management plans; completes standard forms and arranges for services with other community agencies and provincial Ministries such as Income Assistance.
  7. Identifies available social, economic, recreational and educational services and resources in the community to meet client needs and provides client with the information; received client feedback, inquiries and complaints and responds as required; forward more complex issues to the Manager or assigned clinical staff for follow up.
  8. Organizes individual and group client service meetings and schedules community appointments for clinical staff, as required.
  9. Works collaboratively with care team in the documentation of client care plans; completes data entry of care plans and other client service items such as documents and contracts; maintains client records by ensuring that client files, applications forms and financial documents are kept up to date; creates and maintains spreadsheets of client information; completes filing and related administrative duties.
  10. Performs other related duties as assigned.

Qualifications

Education and Experience

Diploma in a social or health services field of study such as Community Social Service and two (2) years' recent related experience working with seniors or adults with physical and/or cognitive disabilities or an equivalent combination of education, training and experience.

Valid BC Driver's License and access to personal vehicle for business related purposes.
