Bring your clinical expertise, dedication and passion for providing quality and compassionate care to a challenging and rewarding role as a PeRiOperative Acute Care Team (PROACT) Nurse Practitioner at the Royal Columbian Hospital (RCH) in the context of team-based care. As B.C.’s oldest hospital, and one of its busiest, RCH provides expert care to the province’s most seriously ill or injured. RCH also provides a number of highly specialized acute care services such as tertiary trauma, internal medicine, cardiology, cardiac surgery, neurosurgery and vascular surgery that often require the care of multiple medical specialists, multidisciplinary care teams and the use of specialized equipment and technology.

 

We have an exciting opportunity for a Nurse Practitioner to join an Internal Medicine consultation service called the PeRiOperative Acute Care Team (PROACT) at the Royal Columbian Hospital. Given the specialized nature of this role, the PROACT service is a collaborative, co-managed, team-based approach to patient care, where the Internists remain MRP or consultant of the patients. This service is available for surgical patients on 3 South, 3 North, 4 North and 4 South. The PROACT team is comprised of internists and a nurse practitioner (NP), who work collaboratively with surgical teams in a consultative capacity with a coordinated approach to ensure safe, quality, effective, and timely care for patients. The goal of this consultation service is to proactively support patient care for optimization while in hospital, thereby improving outcomes and decreasing postoperative complications.

New Westminster is located on the north side of the Fraser River with several bordering municipalities in each direction (Burnaby, Coquitlam, Surrey, and Delta), making it the centre of the Greater Vancouver region. The city is only about 30 minutes (20 km) from Vancouver’s city centre and 30 minutes to Washington state, USA.

 

This position will also have a new and exciting postgraduate up to 6-month nurse practitioner residency program. The Royal Columbian hospital is prime for the NP Residency program due to the multitude of programs and services it offers. You will receive in depth in house advanced training within a clinical area relevant to Internal Medicine, improve your team collaboration and develop a model of patient centred care.  This is a competency-based residency and mentorship program to support the development of new NP knowledge related to the PROACT program and building on competencies for NP practice. FH is the first health authority to implement a formal residency program for NPs in the province, which will be key to supporting the development of NPs and practice-readiness in select programs. As Internal Medicine is a highly specialized area of practice, the residency will consist of dedicated time for both didactic learning and hands-on clinical experiences. The NP will gradually be orientated to the program at RCH while they are completing their residencies.

 

Examples of in-house residency rotations: Internal Medicine, Cardiology, Infectious Disease, Nephrology, Endocrinology, Respirology etc.

You will be welcomed into a New Graduate NP Community of Practice team helping to support your successful transition into independent practice. Be part of our clinical enhancement program with regular in-house skills development such as suturing, Point of care ultrasound (POCUS) and x-ray interpretation in addition to receiving centralized NP orientation.

The Nurse Practitioner is responsible and accountable for the comprehensive assessment of patients/clients/residents including diagnosing diseases, disorders, and conditions. The Nurse Practitioner initiates treatment including health care management, therapeutic interventions and prescribes medications in accordance with the statutory and regulatory standards, limits, and conditions, and employer policies and procedures. The Nurse Practitioner provides professional guidance to other health professionals and practices autonomously and interdependently within the context of an interdisciplinary health care team, making referrals to specialist physicians and others as appropriate.

The position collaborates with patients/clients/residents and other health professionals to identify and assess trends and patterns that have implications for patients/clients/residents, families and communities; develops and implements population and evidence based strategies to improve health and participates in policy-making activities that influence health services and practices. The position participates in peer review and self review to evaluate the outcome of services at the patient/client/resident, community and population level.


Responsibilities

  1. Diagnoses and treats previously undiagnosed patients/clients/residents for undifferentiated diseases, disorders, and conditions within the Nurse Practitioner's scope of practice; writes orders for treatment and medications; provides first line care in emergencies.
  2. Monitors ongoing care, orders appropriate screening diagnostic investigations; interprets reports of investigations and analyzes information to monitor progress and plan treatment.
  3. Establishes priorities for management of health, diseases, disorders, and conditions; provides follow-up treatment; communicates with patients/clients/residents and families about health findings, diagnoses and priorities, outcomes and prognoses; supports and counsels patients/clients/residents in their responses to diseases, disorders and conditions.
  4. Collaborates and consults with physicians or other health care and social service providers as appropriate to assess and diagnose patient/client/resident status.  Develops and implements treatment plans. May admit and discharge patients/clients/residents to facilities according to organizational policies.
  5. Prescribes drugs within the statutory and regulatory standards, limits, and conditions for Nurse Practitioners and within applicable employer policies and procedures. 
  6. Assigns work to other nursing and health care personnel; evaluates work and provides education and supervision as necessary; hires staff, and evaluates staff performance.
  7. Participates in research contributing to improved patient/client/resident care and advances in nursing, health policy development and population health.
  8. Maintains population health focus by implementing screening and health promotion activities for populations at risk.
  9. Participates in interdisciplinary staff and nursing education through case presentations, mentoring, role modeling and facilitating the exchange of knowledge in the classroom, the clinical setting and the community; fosters health care partnerships.
  10. Develops, implements and evaluates policies and procedures related to nursing, interdisciplinary care, and health system practices.
  11. Performs other duties as required.

Qualifications

Education and Experience

Current practicing registration as a Nurse Practitioner with the British Columbia College of Nurses and Midwives (BCCNM).

Recent relevant clinical nursing experience.
