We currently have an exciting opportunity for a Relief FullTime Clinical Social Worker ,Long Term Care to provide services at Bradley Centre-Chilliwack General Hospital located in Chilliwack, B.C. This position is avaialble until June 5th,2026 or until return of incumbent.

Requirements:
- Bachelor’s degree in social work
- One year's recent, related experience in a hospital setting or an equivalent combination of education, training and experience.
- Full registration with the BC College of Social Workers
Detailed Overview
Reporting administratively to the Manager, Clinical Services and clinically to the Clinical Practice Leader, Social Worker, the Social Worker, Long Term Care uses professional and program standards in providing psycho-social assessment, crisis intervention counselling and specific resident treatment, as well as assisting with tasks that are a cross-responsibility of all team members.
Responsibilities
- Facilitates all admissions to the facility by liaising with Access, Home Health and clients; conducts admission interviews and tours; issues medical forms; completes required documentation; schedules admissions and conducts admission conferences for staff.
- Assists residents and family members/significant others with financial matters including equipment acquisition, government applications and other matters as appropriate. Liaises with relevant agencies and inter-disciplinary team members as appropriate in this process.
- Provides counseling, education and support to residents and family members/significant others from pre-admission to death or discharge, as needed.
- Liaises with inter-disciplinary team members and staff; arranges and chairs care plan conferences.
- Maintains records, including statistics, and documents social work services, assessments, interventions in inter-disciplinary focus notes.
- Acts as a resident advocate, maintains confidentiality and practices in an ethical manner, consults with the Professional Practice Chief and other Social Workers on professional practice issues as required.
- Coordinates and chairs Resident Council and Family Support Group meetings; takes minutes and arrranges speakers.
- Participates in the development, implementation and evaluation of professional standards and practices by providing input to the Professional Practice Chief, Social Worker and through Professional Practice Committee activities.
- Participates in continuous quality improvement activities, OH&S and related safety programs, accreditation and regional initiatives as directed by the Manager.
- Performs other related duties as assigned.
Education and Experience
Baccalaureate degree from an approved school of Social Work. One year's recent related experience working with the elderly in residential settings.
Current full registration with the BC College of Social Workers.
