Job Description

The Advanced Practice Clinician (APC) delivers expert-level clinical practice within an interprofessional model of care. In collaboration with Orthopaedic Surgeons and other members of the team, the APC facilitates timely access to quality musculoskeletal (MSK) care and provides referral to appropriate treatment and specialist consultations for patients referred within the Mississauga- Halton (MH) LHIN. The APC is a champion for best practice, and innovative care models and contributes to quality improvement initiatives to achieve safe, effective, efficient care.

The Advanced Practice Clinician has acquired an expert knowledge base and possesses sound decision-making skills in complex situations and clinical competencies for expanded practice. The core function of the position is expert clinical practice that is grounded in theoretical knowledge, advanced education, experience, teaching and research.

This opportunity offers experienced professionals a career progression by working collaboratively with the Orthopaedic surgeons and other members of the interprofessional care team, including APCs within the MH LHIN.

  • Applying advanced clinical skills to perform autonomous comprehensive assessments that incorporate results of the medical history, physical and psychosocial assessment of the health status of patients with musculoskeletal conditions
  • Performing comprehensive musculoskeletal assessment for select patient populations
  • Establishing, communicating and documenting clinical impression within defined scope of practice
  • Initiating communication with and appropriate referrals to orthopaedic surgeons based on assessment finding and referral guidelines
  • Reviewing appropriate diagnostic imaging to support assessment, diagnosis and decision-making
  • Providing side-by-side consultations with orthopaedic surgeons and others on the care team, to establish appropriate care for patients with complex presentations and needs
  • Serving as a clinical consultant for the patient population
  • Providing education and recommendations to patients, families and their primary care providers to support the management of their condition and optimize function in keeping with best practice guidelines
  • Liaising with referral sources and community service providers as required
  • Contributing to the development, evaluation and improvement of team-based care and practice innovations
  • Participating in research to address problems and issues arising from practice
  • Acting as an ambassador for the program

Qualifications
  • Registered Health Professional- Current registration with the applicable college in Ontario - required e.g. College of Physiotherapists of Ontario required
  • Advanced formal education beyond entry to practice training and/or able to demonstrate the ability to work at an advanced level of practice. Examples of advanced formal education include Advanced Clinician Practitioner Arthritis Care (ACPAC) certification preferred
  • Minimum 5 years clinical expertise in the care of arthritis/MSK diagnoses required
  • Completion of the Clinical Practice Skills for Inflammatory Arthritis through The Arthritis Society - Prerequisite for the APC Credentialing Program (in the process or committed to acquiring) required
  • Expert knowledge, complex decision-making skills and clinical competencies to form a foundation for expanded practice required
  • Experience in program development and outcome evaluation required
  • Collaborative approach to teaching/learning required
  • Demonstrated professional leadership, innovation and team building required
  • Demonstrated ability to deal effectively with change required
  • Excellent interpersonal, oral and written communication skills required
  • Satisfactory performance and attendance required

Additional Information

Hours: Monday to Friday, 7.5 hour day shifts (Hours subject to change based on operational needs)

Salary: $47.293 to $62.345 per hour
