Job Description

As a core member of the Out-Patient Cancer Clinic Team your primary focus is to support patients and their families as needed. The social worker will act as a resource to the interdisciplinary team through consultation and education as needed. You will use various modalities including Single Session Therapy, Brief Solution Focused Therapy, individual counseling, family meetings, psycho-educational workshops and education forums. You will provide information and facilitate connections to community resources based on the individual needs of the patient.

  • Conduct comprehensive social work and psychosocial assessments, using a variety of modalities and in accordance with professional standards, clinical policies and practices.
  • Alone or in collaboration with other team members, provide selected therapeutic interventions to patients and families, supportive counseling and education, group interventions, case management, resource coordination, community outreach and advocacy.
  • To collaborate with the inter-professional team to address ongoing patient needs, to participate in regularly scheduled/unscheduled discussions to help optimize patient care and to ensure the coordination of community based support services
  • Intervene to alleviate high risk or vulnerable situations
  • Engage patient to assist with adjustment related to health issues (new diagnosis, illness adjustment, etc.) and identity patient’s network of supports
  • Formulate a plan of action to accomplish identified goals
  • As required, attend rounds to collaborate regarding patient care, information sharing, decision making and enabling patient flow
  • To act as resource liaison to community stakeholders, facilitate referrals to the community and participate in collaborative planning
  • Advocate within both the healthcare setting and the community for the removal of systemic barriers that negatively influence the delivery of health care and the continuity of care of our services to our patients, their families, the organization and our community.
  • Appropriately escalate issues to Social Work and Patient Flow Manager and Director within expected timelines

Qualifications

  • BSW or MSW required.
  • Current registration and membership in good standing with the College of Social Workers and Social Service Workers
  • Minimum 3 years experience in a hospital or community health organization.
  • Previous training/experience with the stages of illness that include diagnosis, recurrence, end-of-life, bereavement and survivorship.
  • Demonstrates strong communication and interpersonal skills with the ability to work collaboratively with patients/families, interdisciplinary team members, and community stakeholders through face to face or telephone interventions, class teaching, group facilitation, adult learning
  • Skillful implementation of planned social work interventions, both individually and in group settings, with the patient, family, caregiver, inter-disciplinary team and community resources
  • Familiarity and/or experience with using evidence-based modalities for treatment such as CB, FBT, DBT and Brief Solution Focused therapy
  • Experience with legislation that governs public hospitals such as the Substitute Decisions Act, Health Care Consent Act, Consent to Treatment Act, Mental Health Act.
  • Demonstrates a high degree of initiative, self-direction and ability to multitask in a fast paced environment
  • Demonstrated ability to function effectively as a member of the health care team.
  • Satisfactory employment and attendance record
  • Comprehensive knowledge of community resources and research skills to secure new resources.
  • Demonstrates strong communication and interpersonal skills with the ability to work collaboratively with patients/families, interprofessional team members, and community stakeholders through face to face or telephone interventions, class teaching, group facilitation, adult learning
  • Care passionately about the needs and well-being of patients, families and caregivers and the needs of our local community

Additional Information

Hours: Monday - Sunday, Days and Evenings, various rotating 7.5 hours (hours subject to change based on operational needs)

Salary: $44.664 - $57.90 per hour
