The Specialized Geriatric Services Case Manager provides in clinic consultations and outreach service in conjunction with Geriatric Psychiatrists and Geriatricians. The Case Manager provides services to patients for therapy service provided by a health professional other than a physician. The Case Manager works with patients, their families and primary care providers to assess and manage complex medical and psychiatric conditions. Outreach visits occur both in patients' homes and in Long-Term Care Facilities. The Case Manager also provides clinical support to the Geriatric Consultation Team across HHS. 


Qualifications
  1. Regulated Health Care professional with current registration in good standing.  
  2. Minimum 5 years of relevant clinical experience.  
  3. Minimum 1 year of demonstrated case management experience. 
  4. Knowledge of Case Manager Theories and Models of Practice.
  5. Demonstrated skill in consultation, assessment, treatment, risk identification and management, counseling and service planning for the elderly.
  6. Demonstrated understanding of the normal aging process; and its impact on functional, cognitive and behavioral ability.  
  7. Demonstrated knowledge of community systems and access to local programs for the elderly. 
  8. Demonstrated knowledge of the impact of illness on clients and their family systems, including the impact on social, ethical, cultural, spiritual and economic factors relevant to the patient population.  
  9. Demonstrated knowledge and experience in providing education to healthcare providers, community partners, clients, family and caregivers. 
  10. Demonstrated ability to develop, conduct and evaluate innovative programs.
  11. Demonstrated excellence in interpersonal skills, collaboration among team members, patient support systems, physicians and community agencies.
  12. Demonstrated enthusiasm for working with an elderly patient population. 
  13. Memberships in gerontological organizations or associations are considered an asset.
  14. Demonstrated ability to establish workload priorities and possess organizational skills required to work in a complex and ever-changing healthcare environment.
  15. Demonstrate computer skills including the ability to effectively use a computerized patient care information system.
  16. A valid Driver’s License and access to a vehicle is required.
