Physiotherapist

As a member of the multidisciplinary healthcare team, the Physiotherapist (PT) will provide patient care through assessment and treatment related to cardiac rehabilitation, chronic disease management and secondary prevention. The Physiotherapist practices in accordance with the Code of Ethics established by the College of Physiotherapists of Ontario. As a resource for the Cardiac Ambulatory Clinics, the PT will support patients receiving care while collaborating with other members of the team, participating in program development, patient education, student supervision, research, and performing other duties as assigned. The PT should be familiar with provincial cardiology guidelines including standards recommended by CorHealth and CRNO.


Qualifications

1. Graduate of an approved university graduate level physiotherapy program (BScPT, MSc. PT) or equivalent.
2. Current Registration in good standing with the College of Physiotherapists of Ontario
3. Current Certificate for Basic Cardiac Life Support (BCLS).
4. Recent (within the last 2 years) experience in cardiac rehabilitation and chronic disease management in an ambulatory setting. 
5. Demonstrated clinical skills and knowledge of functional assessments including those focused on cognition, physical demands, mobility, task analysis and return to work. 
6. Demonstrated clinical skills and knowledge of treatment interventions including energy conservation, ergonomics, stress management, adult learning principles and behavior change concepts.
8. Demonstrated communication skills and the ability to work as part of an interdisciplinary team.
9. Demonstrated knowledge of community partners who focus on health promotion and risk reduction strategies for adults with cardiovascular disease.
10. Demonstrated skills in using computer applications (email, word, excel , Meditech) and with virtual care service delivery though various platforms (OTN, Zoom).
