Job Description: The Assessor Coordinator Senior will assist the client and family to obtain a high quality of care by jointly developing, implementing and evaluating a plan of care that meets the needs of the client and their family/supporters as the client leaves the hospital to continue with care or to support the client to remain independent in the home setting.

Experience

• 1 year exp. working w/ adults living with a disability in community health care
• 1 year exp. working with geriatric clients in community health care preferred.

Education

• Bachelor of Science in Nursing (BScN)

Licenses

• Valid Class 5 driver's license

Other Education and Training

• Bachelor of Social Work is acceptable
• Bachelor of Physical Therapy is acceptable
• Bachelor of Occupational Therapy is acceptable
