Job Description
Under the direction of the Manager and as part of the interdisciplinary Primary Health Care Team, the Assessor Coordinator is responsible for the full scope of case management services in a geographical or program area. Case management is provided to clients receiving services from Home Care, long term care and residents in personal care homes. The core functions of case management practice include referral/intake, engagement, assessment, care planning, monitoring, evaluation, termination/discharge. This is done through collaboration with the family, care givers, and other members of the multi disciplinary team. The Assessor Coordinator liaises with other services in the Regina area and throughout the province, in an endeavor to provide the right service at the right time and in the right place. The Assessor Coordinator works within Primary Health Care and provides one of the entry points for persons needing access to community and acute care services. This position collaborates with acute care, rehabilitation, home care, long term care, mental health and an array of government and non government agencies to assess clients' needs and coordinate services to meet identified needs. Case management is client centered, strength based, flexible, cost conscious and quality driven. One of the key functions of case management is to advocate for and coordinate resources based on assessed need.
Education:
- Recognized degree in nursing or social work.
- Must have a current practicing registration with Saskatchewan Association of Social Workers or the College of Registered Nurses of Saskatchewan.
Experience:
- Eighteen (18) months of recognized clinical experience (postgraduate) preferred
Knowledge & Abilities:
- Required working knowledge of theories, concepts and intervention strategies key for case management practice. Knowledge specific to populations being served including Determinants of health; Cultural issues & diversity in providing care; Impact of disease and illness on quality of life; Mental Health. Cost conscious, quality driven care planning and service delivery. Knowledge of community resources internal and external to the Saskatchewan Health Authority. Client confidentiality and the Health Information Protection Act. Knowledge of Policies, Procedures, Regulations and Legislation pertaining to area of practice. Demonstrated ability to practice core functions of case management (Case finding; Engagement; Comprehensive assessment; Care planning/implementation; Monitoring client service outcomes/evaluation; Reassessment; Disengagement/Discharge).
- Openness to teaching and providing educational opportunities. Empathy and respect necessary to engage, develop rapport and build trusting relationships. Effective interpersonal relationship skills and oral and written communication skills. Advocacy, negotiation, crisis management and conflict resolution. Self-reflective practice, for example: Adhering to professional code of ethics and practice standards. Strong team player with ability to build and maintain stakeholder relationships.
- Standardized documentation which accurately reflects clinical practice.
- Problem-solving and decision-making skills. Time and Organizational management. Manage caseload and work demands. Facilitate case/family conferences. Technical skills necessary to perform functions of position (relevant computer systems).
Other Information
- Preference for this position will be given to HSAS members in accordance with the terms and conditions of the SAHO/HSAS Collective Agreement. HSAS seniority within the Saskatchewan Health Authority will be considered under the Selection Criteria.
- This is a community based position providing services in clinic and home settings. Travel required to various community sites, including rural, within the Primary Health Care Networks (Regina). Home base for this position is: 4211 Albert Street.
- Must have a valid driver's license. Some assignments may require access to a vehicle for work.
