Job Description: The overall function of the assessor/coordinator (client care coordinator) position is to assess and coordinate single entry access to long term care, respite and community day programs.

Education
- Bachelor of Science in Nursing (BScN)
Other Education and Training
- Bachelor of Occupational Therapy is acceptable
- Bachelor of Social Work is acceptable
- Nursing diploma is acceptable
- Bachelor of Physical Therapy is acceptable
Knowledge and Abilities
- Knowledge in the SHA, community resources and applicable programming
